노년기 우울 치료를 위한 심리사회적 개입의 효과성: 체계적 검토방법을 중심으로 = A Systematic review for the studies of the psychosocial interventions for the depressed elderly in South Korea

The purpose of this review article was to assess the available scientific evidence on the effects of psychosocial interventions for the elderly with depressive symptoms in South Korea. For the present study, this study conducted an extensive search of three electronic databases using a comprehensive search strategy. The literature search identified 16 studies evaluating psychosocial intervention for older adults with depressive symptoms. In results, even though all reviewed studies reported overall effects of each intervention, this review found that 1) they were lack of enough evidences in terms of the scientific rigorousness; 2) although the purpose of the intervention studies were decreasing depression or depressive symptoms of older adult population, about a half of the samples were not from the persons with depressive symptoms; 3) most of the reviewed studies were based on the specific theories, but some of the studies showed conceptual issues; and 4) most of the reviewed studies were based on a group treatment, and thus individual interventions were a few. Through a systematic review for drawing the possible evidence of psychosocial intervention for depressed elderly in this study, this study suggested some points for the future psychosocial intervention studies for depressed older adults in Korea.

국문 초록 (Abstract)

본 연구의 목적은 현재까지 국내에서 이루어진 노인 우울 완화 및 치료를 위한 심리사회프로그램의 효과성을 체계적으로 검토(systematic review)하는 것이었다. 본 연구 목적을 달성하기 위해 국내의 주요 논문검색 엔진을 통해 총 16편의 논문을 추출, 분석하였다. 모든 연구들은 개입 대상 및 표본 수, 연구 설계, 구체적 개입유형, 개입의 내용, 측정도구, 결과의 점검방식 및 효과성 등이 검토되어졌다. 분석결과 첫째, 분석대상 모든 논문이 개입 직후 및 개입완료 2-3개월 이후 시점에서 효과성을 보고했으나 효과성을 입증할만한 과학적 엄격성은 부족했다. 둘째, 개입의 목적이 노인의 우울증상의 치료 및 완화임에도 불구하고 표본을 일반 노인층을 대상으로 한 연구가 거의 절반이어서 조사대상 선정의 적절성에 문제를 드러냈다. 셋째, 대부분의 연구가 특정 이론을 기반으로 개입프로그램을 구성했으나 일부 연구는 이론에 대한 면밀한 검토가 부족한 것으로 보인다. 넷째, 개입의 유형에 있어서도 집단프로그램이 절대 다수를 차지했고 개인 대상 개입은 고작 3편 밖에 되지 않아 개입 유형의 편중이 심했다. 이 같은 연구결과를 통해 향후 노인 대상 우울 치료를 위한 심리사회적 개입의 방향이 제시되었다.
