노인돌봄 공적재가서비스 이용에 따른 노인과 가족 간 관계 변화에 대한 사례연구 = Case Study on Changes in Relationship between Family Care givers and Care receivers Arising from Public Home Care for the Elderly

국문 초록 (Abstract)

본 논문은 노인돌봄을 둘러싸고 노인과 가족 간 (권력)관계에 어떠한 변화가 나타났는가, 특히 공적 재가서비스 이용이 돌봄수혜 노인과 가족돌봄제공자 간 관계에 어떠한 영향을 미치고 있는가 라는 점에 연구의 초점을 두고 작성되었다. 분석의 핵심은 노인돌봄을 둘러싸고 노인 돌봄수혜자와 가족 돌봄제공자 간 권력관계가 사회적 지원에 의한 권력자원의 변동으로 어떻게 변화하는가이다. 즉 노인돌봄을 둘러싼 가족 간 상호작용, 부담과 갈등이라는 개인적, 가족적 맥락이 노인돌봄정책 도입을 통한 사회적 맥락의 변화에 의해 돌봄수혜 노인과 돌봄제공 가족 간 관계적 자율성에 어떠한 영향을 미치고 있는지를 개별 심층사례 연구를 통하여 분석하고자 하였다.
따라서 본 연구는 돌봄수혜자와 (가족)돌봄제공자의 이자적 관계 뿐만 아니라, 돌봄을 사회적으로 조직하고 지원하는 사회적 맥락(돌봄규범 및 돌봄정책)을 중요한 요인으로 고려하였다. 즉, 노인돌봄의 사회적 맥락내에서 돌봄수혜 노인과 (가족)돌봄제공자 간의 권력 관계 변화에 초점을 두었다.
본 연구 결과, 돌봄을 조직하는 사회질서, 즉 사회의 돌봄규범과 돌봄정책을 통한 공적지원에 따라 노인의 권력자원은 달라지고, (가족)돌봄제공자와의 권력관계에서의 협상력은 달라짐을 발견하였다. 즉, 돌봄정책을 통해 노인의 가족에 대한 의존성 약화로 노인의 돌봄협상력이 제고될 뿐 아니라 가족의 대안적 선택권의 제고로 가족의 돌봄협상력도 제고될 수 있다.

다국어 초록 (Multilingual Abstract)

The objective of the Paper has been focused on the consequences of public elderly home care policy on the changes in the relationship between family care providers and care receivers in the wake of the emergence of elderly care needs and implementation of home care policy. This Paper has attempted to analyze the possible changes in rational autonomy of care givers and care receivers such as burdens and conflicts on the part of care givers of elderly home care from the social context of individual and family care givers through the analysis of various individual in-depth case studies.
The core concept of analysis has been centered on the consequence of social assistance on the changes on power resources among members of family. Through this analysis, the changes in rational autonomy (such as their negotiation positions and choice options) of family members participating to the care setting as care receiver and care givers have been studied.
Accordingly, the key elements of this Study include not only the bilateral relationship between care receiver and care giver but also include social framework (such as care norm and policy) that assists and constraints home care policy from social context. In this regard, the focus of this Study has been centered on the changes in power resources of care receiver and care giver from the social context of elderly home care. The results of this Study confirmed that public elderly care policy has brought about substantial changes in the rational autonomy. The elderly care policy enhance the negotiating position of care receivers by mitigating their family dependence while the alternative choices of family care givers were also enhanced by increasing choice possibilities.
