연구목적본 연구는 외상 후 스트레스 장애 환자(Post-traumatic stress disorder, PTSD) 중 Clinician-Administered PTSD Scale (CAPS) Minnesota Multiphasic Personality Inventory-2 (MMPI-2)를 시행한 환자들을 대상으로 피해 사고에 영향을 미치는 요인을 확인하고자 한다.
방 법2013년 5월부터 2020년 4월까지 중앙보훈병원에서 CAPS와 MMPI-2를 시행한 116명의 환자를 후향적으로검토하였다. CAPS 점수를 기준으로 하여 환자들은 PTSD군(n=63, 연령: 58.16±17.84)과 외상 노출이 있는 비PTSD군(n=53, 연령: 67.34±12.05)으로 나뉘었다. 피해사고, CAPS 및 MMPI-2의 척도들 간의 상관 관계를 확인한 후 임상적으로 관련된 위험 요인을 확인하기 위해 선형 회귀 분석을 시행하였다.
결 과PTSD 군은 외상 노출이 있는 비PTSD 군에 비해 MMPI-2에서 정신분열증, 피해 사고, 역기능적 부정적 정서, 기태적 경험, 정신증, 부정적 정서성/신경증, 불안, 우울, 분노 척도에서 유의한 차이를 보였다. 피해 사고와CAPS, MMPI-2의 상관 관계를 보았을 때, 피해 사고와 MMPI-2 항목들 간에는 통제 결여 항목을 제외하고강한 관련성을 보였다. PTSD 군에서 다중선형회귀분석을 통해 역기능적 부정적 정서 및 분노 척도가 피해 사고의 위험 요인임을 확인하였다.
결 론PTSD 군은 외상 노출이 있는 비 PTSD 군에 비해 피해 사고가 높게 나타나고, 역기능적 부정적 정서 및 분노척도가 피해 사고의 위험 요인일 수 있어 이에 대해 평가하고 적절한 치료를 제공하는 것이 필요하다.

Objectives:The aim of this study was to identify the factors affecting ideas of persecution in post-traumatic stress disorder (PTSD) patients who underwent Clinician-Administered PTSD Scale (CAPS) and Minnesota Multiphasic Personality Inventory-2 (MMPI-2).
Methods:We retrospectively reviewed 116 patients who underwent CAPS and MMPI 2 between May 2013 and April 2020 at Veteran Health Service Medical Center. Based on the CAPS score, the patients were divided into the PTSD group (n=63, age: 58.16±17.84) and the trauma exposed without PTSD group (n=53, age: 67.34±12.05). After checking the correlation between Ideas of persecution, CAPS, and MMPI-2 scales, linear regression analysis was performed to identify the risk factors for clinically relevant symptoms.
Results:The PTSD group showed significant differences in Schizophrenia, Ideas of persecution, Dysfunctional negative emotions, Aberrant Experiences, Psychoticism, Negative Emotionality/Neuroticism, Anxiety, Depression, and Anger scales compared to the trauma-exposed without PTSD group. When analyzing the correlation between Idea of persecution, CAPS and MMPI-2 scales, there was a strong association with most of the scales in MMPI-2 and Idea of persecution except Disconstraint. Multiple linear regression analysis performed in PTSD group identified that risk factors for Idea of persecution were Dysfunctional negative emotions and Anger scale.
Conclusions:The PTSD group had increased idea of persecution compared to the trauma exposed without PTSD group. Dysfunctional negative emotions and anger may be risk factors for idea of persecution in trauma exposed population.
