저소득층 노인과 방문 돌봄 서비스 종사자의 유사가족관계 연구 = A Study on Quasi-Familial Relationships between Home Care Workers and Their Low-Income Elderly Clients

국문 초록 (Abstract)

  본 연구에서는 방문 돌봄 서비스를 받는 저소득층 노인과 종사자 간의 돌봄을 매개로 한 유사가족 관계를 연구하였다. 질적 연구방법을 이용하여 돌봄 대상 노인들과 종사자들에 대한 개별 심층면접과 초점집단 면접을 수행한 결과, 이들은 서로 일상과 감정을 공유하면서 친밀한 관계를 형성하고, 돌봄 종사자들이 가족원과 유사한 돌봄의 역할과 기능을 수행하고 있는 것으로 나타났다. 유사가족관계는 돌봄 대상노인에게는 생활 전반에 대한 지지를, 돌봄 종사자에게는 보람과 만족을 제공하는 상호호혜적인 성격을 갖고 있었다. 그러나 일상생활 전반에 대한 개입과 의존, 이로 인한 종사자의 심리적 부담과 근무 외 돌봄 수행, 관계의 단절에 대한 불안감 등의 문제점도 나타났다. 방문 돌봄 서비스 관계에서는 도구적 서비스 제공보다 돌봄 제공자와 노인 간의 정서적 유대와 친밀감이 더 중요한 의미를 갖는 것으로 나타났다. 공적 돌봄 서비스를 매개로한 유사가족관계는 가족에 의한 노인 돌봄을 이상화하는 가족 규범과 공적 돌봄 서비스와 관련된 편견이 현실에서 재구성내지는 부정되고 있음을 의미하며, 궁극적으로 노인 돌봄 정책과 서비스에 내재된 가족주의가 변화되어야 함을 보여준다. 방문 돌봄 서비스의 질을 향상시키기 위하여 서비스 관계의 지속성을 보장하고 종사자의 권리를 법적으로 보호하며 돌봄 노동의 사회적 위상을 제고할 것을 제안하였다.

다국어 초록 (Multilingual Abstract)

  The study investigated quasi-familial relationships between in-home service workers and their low-income elderly clients. The data was drawn from in-depth interviews with 11participants including both party of care providers and recipients, and focus group interviews with 4 workers. As the results of the analysis on the data, it was shown that they developed personal ties by sharing daily lives and emotions. In particular, the workers performed diverse roles of family for the elders. Public home care service and family care were most similar in the scope and nature of the work in the relationships. The intimate relationships can be called "reciprocal" in that the workers acquire emotional compensation such as a sense of meaning and vocational satisfaction, while the elderly clients are able to maintain a sense of being supported and cared for. The problems of the quasi-familial relationships are: excessive demands and dependency of the elderly clients; workers" feeling of burden; extra work than the paid hours. The emotional intimacy between the elderly clients and workers appears to be more important than instrumental or physical services in community based care services because of the characteristics of home care recipient elders such as relatively independent functional status and lack of personal contacts. The results of the study indicate the necessity of revising the family ideology which idealizes elder care performed by family members and the prejudice concerning the poor quality of emotional care of public services. For the future development of in home care services, policy suggestions were made including assuring the continuity of relationship in organizing care services, promoting social status of home care workers, and protecting their legal rights related to work hours and boundaries.
