ESD는 병변의 위치, 크기 및 모양과 상관없이 절제가 가능하고, 점막하층까지 정확하게 박리하여 종양을 일괄 절제할 수 있으며, 이는 기존 내시경 점막 절제술(endoscopic mucosal resection)에 비해 병리 평가가 용이하고 재발 위험이 현저히 낮다는 이점을 제공한다[2-4]. 이러한 장점에도 불구하고 ESD는 넓은 점막 및 점막하층 결손을 남기기 때문에 시술 직후 거대한 인공 궤양(artificial ulcer)이 형성된다. 이러한 인공 궤양은 일반적인 소화성 궤양(peptic ulcer disease)과는 다른 기전과 발생과정, 병태생리, 임상경과를 보이며, 이로 인해 시술 후 출혈 또는 드물게 천공과 같은 합병증 위험이 존재한다[5].
ESD 후 발생하는 인공 궤양은 고주파 절제 과정에서 발생한 응고 괴사층, 광범위한 점막하층 노출, 근육층 일부 가열 손상, 혈관 구조의 취약화 등의 요소로 인해 치유가 지연되며, 특히 시술 후 24시간에서 2주 사이에 발생하는 지연 출혈(delayed bleeding)은 ESD 후 가장 중요한 합병증 중 하나다. 실제로 여러 대규모 연구에서 ESD 후 지연 출혈률은 2–8%로 보고되며, 고령 환자, 항혈전제 복용 환자, 절제 범위가 넓은 환자에서 더욱 증가한다[5-7].
ESD 시술 증가에 따라 ESD 후 발생하는 인공 궤양의 관리에 대한 관심이 높아지고 있으며, 현재 표준 치료법으로 사용 중인 위산 분비 억제제와 같은 약물 치료뿐 아니라, 폴리글리콜산(polyglycolic acid, PGA) sheet, fibrin glue, 내시경적 봉합술(endoscopic closure), 인공지능(artificial intelligence)을 활용한 출혈 예측 모델 등 새로운 치료 전략이 등장하고 있다. 본 종설에서는 ESD 후 발생하는 인공 궤양 치유의 병태생리적 이해를 기반으로, 치유 과정, 예후 결정 요인, 약물 치료 방법, 최신 연구 동향 및 향후 전망까지 폭넓게 고찰하고자 한다.

Endoscopic submucosal dissection (ESD) has become the standard treatment for early gastric neoplasms owing to its high en bloc resection rate and low recurrence. However, ESD inevitably creates large artificial ulcers, which differ markedly from conventional peptic ulcers in etiology, morphology, and healing behavior. Post-ESD ulcers develop rapidly through thermal injury, leaving extensive mucosal and submucosal defects while preserving the muscularis propria.
Healing is characterized by early ulcer contraction followed by epithelial regeneration, typically achieving over 90% closure within 4 weeks and complete reepithelialization by 8 weeks.
Despite this, delayed healing occurs in 5–20% of patients. Major risk factors include Helicobacter pylori infection, large ulcer size, antral location, and high-grade dysplasia or early gastric cancer. Acid suppression remains the cornerstone of therapy. Proton pump inhibitors promote healing through potent acid inhibition and cyclooxygenase-2-mediated mucosal regeneration, whereas potassium-competitive acid blockers provide faster, stronger, and cytochrome P450 2C19-independent acid suppression, offering advantages during the early high-risk period.
Mucoprotective agents, such as rebamipide and sucralfate, further enhance epithelial repair and reduce mechanical or chemical irritation. Endoscopic closure techniques―including clipping, use of over-the-scope devices, loop-clip methods, and the application of polyglycolic acid sheets with fibrin glue―offer additional protection and help reduce delayed bleeding. Emerging advances include artificial intelligence-based prediction models for delayed bleeding, biomaterialassisted tissue regeneration, and precision treatment tailored to genetics, ulcer characteristics, and patient-specific risk factors. Collectively, these developments are reshaping post-ESD ulcer management toward more individualized and proactive care.
