    위암은 한국에서 2024년 암유병율 2위, 암사망률 4위를 차지하는 질환이다[1]. 조기 위암 진단의 비율이 증가하면서 내시경 절제가 증가하고 위절제술은 감소하고 있으나 여전히 한국에서 1년에 1만 건 이상의 위절제술이 시행되고 있다. 2019년 한 해 동안 위암(adenocarcinoma)으로 수술을 받은 환자는 14,076명[2], 대한위암학회(Korean Gastric Cancer Association)의 2023년 전국 설문조사에 의하면 66개 기관이 총 12,751건의 위암 수술 사례를 보고하였다[3].
    위절제술 후 위암 환자에서 영양상태는 종양 병기와 무관하게 장기 생존을 결정하는 독립적인 예후 인자로 확인되고 있다. 특히 수술 전·후 영양불량, 근 감소(sarcopenia)등은 합병증 증가뿐 아니라 전체 생존(overall survival, OS) 및 무병생존(disease-free survival, DFS) 의 유의한 감소와 연관된다. 최근 연구에서는 조기·충분한 영양지원과 퇴원 후 지속적 영양관리가 생존 향상 가능성을 보여, 영양 관리가 위암 치료의 핵심 요소로 부각되고 있다.
    본 종설에서는 위 절제 환자에서 영양 관리에 대해 정리해보고자 한다.

    Despite decline in frequency of gastrectomy due to early detection of gastric cancer, surgery necessitates in many cases, thereby making preoperative nutritional assessment essential. Recovery and postoperative nutritional status are well known to influence prognosis. Anatomical and physiological changes following gastrectomy impair digestion and absorption. Loss of reservoir function and reduced gastric accommodation lead to early satiety after small meals, decreased food intake, and resultant weight loss. Moreover, decreased gastric volume and parietal cell loss cause iron malabsorption, while intrinsic factor deficiency impairs vitamin B12 absorption. Delayed mixing of bile and pancreatic secretions decreases fat absorption, and reduced gastric acid secretion delays protein digestion, contributing to metabolic disturbances. Upon gastrectomy, the reduced food intake, malabsorption, and hormonal changes collectively increase the risk of sarcopenia. Loss of lean body mass is associated with long-term deterioration in quality of life, as measured by the PGSAS-45. Reduced gastric acid also impairs calcium ionization, lowering absorption efficiency, while decreased intestinal vitamin D absorption contributes to long-term disturbances in bone metabolism. A retrospective study of patients undergoing laparoscopic radical gastrectomy demonstrated that the preoperative prognostic nutritional index (PNI) was associated with both postoperative complications and long-term outcomes. Patients with good nutritional status demonstrated a favorable long-term prognosis even when complications occurred, whereas those with low PNI exhibited poor long-term outcomes in the presence of complications. These findings underscore the critical role of nutritional assessment and management in optimizing recovery and prognosis after gastrectomy for gastric cancer.
