질병관리본부 건강감시체계를 기반으로 한 국민건강증진 종합계획 목표지표 모니터링 체계 개발 연구 = Health People 2010 objectives monitoring system development based on KCDC health surveillance system

o The objectives of this study were to evaluate of achievement of HP2010 by using KCDC health surveillance system and to develop HP2010 objectives monitoring system based on KCDC health surveillance system.
o We analysed database of KCDC health surveillance system and non-KCDC database to determine which data of objective can be produced. Also we evaluated a current achievement status of HP2010. We comprehensively reviewed domestic and foreign studies about health plan monitoring system and operated expert consultation. By doifig this, we suggested some reformation of HP2020 model and monitoring and evaluation system about HP2020.
o After analysing production possibility of objective, production impossibility were 26.0%. Standard which cannot be measured were 22.8% of all objectives.
o We can produce 68.0% of all measurable data from KCDC health surveillance system and 32.0% from another non-KCDC database.
o In the case of HP2010 achievement, 44.4% of all standard were improved or changed 10% from year 2005.
o Monitoring and evaluation model of HP2020 wad made with reflecting multiple dimension of HP2010, for example, domain, intended user, life course, health equity, interval of monitoring. We divided domain by determinant, intervention and outcome. For example, determinant were smoking, alcohol drinking and outcome domain contained health prevention management. We didn’t have intervention in HP2010, so we arranged infrastructure, educational program, ease accessibility and others at intervention domain.
o For smooth and successful implementation of suggested monitoring system, we must collect data from multiple database and coordinate management of database. Also we need the HP committee for continuous HP monitoring and evaluation. And operation of the monitoring and evaluation system must be based on the legal ground. We suggested an establishment of monitoring system within responsibility of government.

국문 초록 (Abstract)

o 연구의 목적은 질병관리본부의 건강감시체계를 활용하여 HP2010의 성과를 평가하 고 질병관리본부의 건강감시체계에 기반을 둔 HP2020 목표지표 모니터링 및 평가체 계를 개발하는 것임.
o 연구 목적 달성을 위해 질병관리본부 건강감시체계와 질병관리본부 이외의 기관이 주관하는 DB의 구조 및 산출자료를 분석하여 HP2010의 목표 지표의 산출 가능성을 검토하고 달성 현황을 평가，이를 통한 HP2020 모형의 개선방향을 제안함. 외국문헌 검토와 전문가 자문 시행, 제안된 HP2020 모형을 통해 HP2020 모니터링 및 평가 체 계를 생성하고 HP2010에 대한 모니터링 모형 적용을 실시함.
o HP2010의 목표지표 산출 가능성을 분석한 결과 총 169개의 목표지표 중 44개의 목 표지표는 산출할 수 없어 산출 불가능성은 26.0%였음. 그리고 촉정대상 지표 219개 중 50개는 측정할 수 없어 측정 불가능성은 22.8%였음.
o 산출 자료원 분석 결과 측정 가능한 지표들의 자료원을 질병관리본부에서 산출 할 수 있는 지표와 그렇지 않은 경우로 나누어 볼 때, 질병관리본부에서 생산가능 지표 는 총 측정 가능 지표 169개 중 115개(68.0%), 나머지 기타 자료를 통하여 54개 (32.0%)를 산출 할 수 있었음.
o 전체 총 169개 측정 가능 지표의 달성도를 분석 시 54개 기타 자료(32.0%)에서의 달 성도를 알 수 는 없지만 115개 질병관리본부의 자료(68.0%)에서 보았을 때 변동이 2005년도 기준 10%이내 이거나 개선된 지표들이 전체 지표 중 444%를 차지하고 있음.
o HP2020 모니터링 모형은 영역, 활용주체，생애주기, 형평성，모니터링 주기 등의 다 양한 dimension을 반영하는 구조로 생성되었음. HP2아0의 4개 중점분야 중 ‘건강생활 실천 확산’에 해당하는 금연，절주 등의 중점과제들은 Determinant 영역으로, ‘예방중 심 건강관리’에 해당하는 암관리，심뇌혈관질환 등의 중점과제들은 Outcome 영역으로 분류하였으며 Intervention 영역에 해당하는 중점과제는 양질의 의료에 대한 접근성， 교육 및 지역사회 프로그램, 건강정보 접근성，공중보건 인프라 등으로 구성할 수 있 을 것임.
o 제안된 모니터링 체계의 원활하고 성공적인 진행을 위해서는 HP 목표지표의 결과 값 산출에 필요한 다양한 자료원으로부터의 정보 수집과 자료원에 대한 통합적 관리, HP 모니터링 및 평가를 위한 HP 위원회 구성과 운영이 필요함. 또한 이를 뒷받침 할 수 있는 HP 목표지표 모니터링 및 평가체계와 관련된 법적 근거 마련이 필요함. HP에서 달성하고자 하는 최종 목표와 목적들이 구현될 수 있도록 정부의 책임성이 제고될 수 있는 제도 마련이 동반되어야 함.
