선원 항생제 사용 실태와 선박 약물 관리 개선을 위한 기초 연구 = A Preliminary Study on Antibiotic Use among Seafarers and Improvement of Onboard Medication Management

국문 초록 (Abstract)

본 연구는 국내 선원을 대상으로 항생제 자가투여의 실태와 선박 내 약물 관리의 개선 필요를 파악하였다. 단면 설문을 통해 선원 현장의 의사결정 맥락을 수집·분석한 결과, 자가투여가 빈번하고 그에 따른 부작용 위험과 관리공백이 확인되었으며, 주요 원인은 경미 증상 시의 관성적 복용, 의료접근성의 제약, 선내 비치약 의존으로 요약되었다. 현 체계는 교육·지침·연락체계·통신 인프라 전반에서 보강이 요구되며, 행동 설계에 기반한 교육과 표준화된 원격자문 프로토콜, 비치약 관리의 체계화가 핵심 해법으로 제시하였다. 본 연구를 통해 저자는 행동 설계형 교육 콘텐츠와 증상 기반 의사결정 도구의 공동 설계자, 선사·현장·보건당국을 잇는 실무 연계 촉진자로서의 역할을 수행함으로써 연구–현장–정책의 순환적 피드백을 구축하고, 선원 약물 안전을 실질적으로 향상시키기 위한 기초자료를 제공하고자 한다.

This study investigates antibiotic self-medication among Korean seafarers and the need to strengthenonboard medication management. Using a cross-sectional survey that captured real-world decision contexts, Ifound frequent self-medication, elevated adverse-event risk, and clear management gaps driven by habitual usefor “mild” symptoms, constrained access to care, and reliance on ship-stock medicines. System upgrades arewarranted across training, guidance, teleconsultation links, and communications infrastructure; accordingly,behaviorally informed education, standardized telemedical consultation protocols, and systematic control ofonboard stocks are proposed as core solutions. As the sole author, I position myself not only as a reporter offindings but as a co-designer of behavior-based training content and symptom-driven decision tools, and as apractice–policy bridge linking shipping companies, crews, and health authorities—thereby establishing a feedbackloop between research, practice, and policy and providing foundational evidence to improve medication safety forseafarers.
