    학동기 아토피 피부염 환자에서 맞춤형 식사를 통한 증상 호전 = Beneficial Effect of Diet Intervention on School Children with Atopic Dermatitis

    Background: Atopic dermatitis (AD) is one of the most common skin disorders affecting infants and children.
    Food allergy is a strong risk factor for the development of AD. This study was designed to evaluate the improvement of symptoms among AD patients in the late childhood while they take the customized (organic, balanced, non-allergic, diverse vegetable, little saturated fat and trans fat free) diet.
    Methods: Nineteen children with AD who agreed to the written consent were enrolled in the study from October to December, 2014. We supplied them with the customized diet for 8 weeks. Clinical evaluations, blood tests and nutritional status were conducted at baseline, at 4th and at 8th week. Eczema area and severity index (EASI) and Scoring atopic dermatitis (SCORAD) were used to determine the severity of AD.
    Results: The mean age of 19 patients was 8.5±1.54 years. EASI and SCORD index were decreased after diet intervention (2.6±3.06 [standard deviation] vs. 2.0±2.93 P=0.026, 24.9±10.76 vs. 20.0±12.65 P=0.009).
    Pruritus scores using visual analogue scale, rated by patients and hemoglobin revealed statistically significant improvement after diet intervention.
    Conclusions: The customized diet helps to improve the symptoms of atopic dermatitis and the nutritional indicators. Additional long-term and large-scale studies are required as well as the development of various diets.

    연구배경: 아토피 피부염의 병인은 아직 명확히 밝혀지지않았으나 유전적 원인, 면역학적 이상, 표피 지방대사이상등이 알려져 있다. 특히 영유아기와 소아기에는 음식 항원또한 아토피 피부염의 발생 및 악화에 중요한 역할을 한다고 알려져 있다. 또한 아토피 피부염 환아는 음식물 섭취의과도한 제한으로 성장장애와 영양소 결핍이 흔히 관찰된다. 따라서 본 연구에서는 학동기 아토피 피부염 환아에서흔한 원인식품을 제거한 맞춤형 식단을 제공한 후 아토피피부염의 호전 양상을 알아보고자 한다.
    방법: 2014년 10월부터 12월까지 8주 동안 대구지역 일부초등학교의 아토피 피부염 환자 중 연구 참여에 동의한 19 명을 대상으로 하였다. 8주 동안 맞춤형 식단을 제공하였으며 식단 제공 전 후 기본신체검사, 혈액검사, 영양상태를 평가하였다. 아토피 피부염의 중증도는 EASI, SCORAD를 이용하여 평가하였다.
    결과: 19명 환아의 평균 나이는 8.5±1.54세였고, 남녀 각각 10명, 9명이었다. 아토피 피부염의 중증도는 식사 전후EASI 점수가 2.6±3.06에서 2.0±2.93으로 감소하였고(P= 0.026), SCORAD 점수는 24.9±10.76에서 20.0±12.65로 감소하였다(P=0.009). 환자의 주관적인 증상은 소양감에 대한 visual analog scale 평균값이 4.4±1.79에서 3.6±2.01로 유의하게 감소하였다(P=0.018). 혈액검사상 식사 전후 혈색소 평균값이 13.1±0.78 g/dL에서 13.6±0.69 g/dL로 유의하게 증가하였다(P<0.001).
    결론: 저자들이 개발한 아토피 피부염 맞춤형 식단은 아토피 피부염 환아들의 아토피 피부염 증상과 영양지표 호전에 도움이 되었다. 추가적인 장기간 및 대규모 연구가 필요하다.
