공항장애와 주요우울장애의 관계 = The Relationship between Panic Disorder and Major Depressive Disorder

국문 초록 (Abstract)

공황장애와 우울증에 대한 생물학적 지표는 각각 Table 4, 5, 6,에 나타나있다(Nathan과 Schatzberg 1994 ; Papp등 1994). 이처럼 공황장애와 주요우울증의 관계를 명확하게 하기 위한 연구가 많이 행해졌다. 여러 측면에서 공황장애와 주요우울증은 동일한 질환이 아니다. 많은 공황장애 환자가 일생 동안 우울증을 경험할 가능성이 있지만 그 시기는 매우 다양하며, 우울증이 만성불안과 공포성 회피에 의한 결과라고 예상할 수는 있지만 그 가설을 지지하는 증거는 없다. 우울증은 심한광장공포증이 있건 없건 발생할 수 있고 얼마동안 공황장애를 않은 사람에게도 발생할 수 있다.
공황장애에 대인공포증이나 강박증 같은 질환의 동반은 우울증 발생의 위험요인이며 더 심한 질환을 나타낼 수 있다. 임상에서 실제로 진단에 도움되는 것은 제한되어 있지만 생물학적 지표(Table 7)는 공황장애와 주요 우울장애 사이의 중요한 병태생리학적 유사점과 차이점을 나타낼 수 있다.
현재 증거는 일부 독립된 중요영역과 더불어 생물학적으로 중첩되는 많은 영역을 가리킨다(Fig. 7A). 비록 여기서 많이 논의되지는 않았지만 몇몇 임상적 매개변수는 비슷한 양상을 나타낸다.(Fig. 7B).
결론적으로 우리는 공항장애와 주요우울장애가 많은 특징들을 공유한 서로 다른 질환이라는 개념화에 찬성한다. 미래에는 유전학과 함께 결국 이들 두 흥미로운 정신과적 증후군 사이의 경계를 더욱 분명하게 할 것이다.

다국어 초록 (Multilingual Abstract)

Frequently, the course of panic disorder is complicated by the occurrence of depression, and many patients with a mixed anxious/depressed condition. Similarly, many patients with major depressive disorder have panic attacks and other prominent symptoms of anxiety.
Despite this frequent overlap between anxiety, especially panic disorder, and depression, little is known about pathophysiologic factors of potential relevance to this overlap.
So l addressed the issues of lifetime prevalence of depression in patients with panic disorder, phenomenoiogy, biologic probes into various neurotransmitter systems, and neuroendocrine findings through several references.
Conclusively, panic disorder and major depressive disorder are nonidentical disorders with many shared characteristics. And in the future, genetic studies may eventually lead to a clearer demarcation of the boundaries between these two intriguing psychiatric syndromes.
