Nocturnal paroxysmal dystonia(NPD) is a syndrome consisting of recurrent motor attacks with dystonic-dyskinetic features arising from non-rapid eye movement(NREM) sleep. There has been controversies about the disease entity, epilepsy or movement disorders. Recently it is considered as a form of nocturnal frontal lobe epilepsy. The author reports the clinical, EEG and ictal SPECT findings of a patient with nocturnal paroxysmal dystonia. The ictal SPECT showed increased uptake on the mesial frontal region. These findings support the earlier report the NPD as frontal lobe epilepsy.
