노인의 건강정보이해능력(Health Literacy)에 영향을 미치는 요인 분석 : 앤더슨 행동모형 적용 = A Study on Predicting Health Literacy of Korean Elderly Using Andersen’s Health Behavior Model

This study aimed to identify the level of health literacy and associated factors among Korean older adults using Andersen’s health behavior model. We collected data in October-November, 2012, using a self-administered questionnaire. 640 older adults aged 65 and older were sampled and a total of 596 subjects were analyzed for further analyses. The following influencing variables were selected based on Andersen’s behavioral model of health literacy: predisposing factors(age, gender, marital status, place of residence), enabling factors(education level, literacy level, family income, job, social support), and need factors(self-rated health status, number of chronic comorbidities). Using a hierarchical logistic regression model, we found that age and single status among predisposing factors were inversely associated with health literacy. For enabling factors, educational attainment, literacy level, and family income had positive relationships with health literacy, when controlling for other pertinent factors. This study suggests that community-based health literacy programs should prioritize and educate those with a lower level of health literacy.

국문 초록 (Abstract)

본 연구는 Andersen 행동모형을 적용하여 노인의 건강정보이해능력 수준을 파악하고 이에 영향을 미치는 요인을 분석하고자 한다. 본 연구는 전국 노인 596명을 대상으로 자기기입식 설문조사를 통해 자료를 수집하였다. 노인의 건강정보이해능력에 영향을 미치는 요인을 분석하기 위해 소인성 요인(성별, 연령, 배우자 유무, 거주지역), 가능성 요인(교육수준, 문해능력수준, 월평균가구소득, 취업여부, 사회적지지), 필요성 요인(주관적 건강, 만성질환 개수, 우울수준)을 독립변수로 선택하여 위계적 회귀분석을 실시하였다. 노인의 건강정보이해능력에 영향을 미치는 요인으로 소인성 요인으로 연령이 높을수록, 가능성 요인에서는 교육수준이 높을수록, 기초문해력이 높을수록 건강정보이해능력 수준이 높은 것으로 나타났다. 필요성 요인에서는 만성질환 개수가 많을수록, 우울수준이 낮을수록 건강정보이해능력 수준이 높은 것으로 나타났다. 본 연구결과를 토대로 노인의 건강정보이해능력 제고를 위해 정책적 개입의 필요성이 높은 대상을 발굴하고 핵심적인 개입요인을 발굴하여 노인의 낮은 수준의 건강정보이해능력 문제를 극복할 수 있는 국가차원의 정책프로그램을 개발하는 것이 필요하며, 이를 위해 고려해야 할 사항들을 제시하였다.
