This study targeted elderly people with chronic disease between Class 1 and Class 7 of national meritorious persons, who are being admitted to the community welfare center and the VHS Medical Center. It verified the influence upon disease-coping level in the elderly, the mediation effect of the disease-coping level in the relationship among uncertainty of disease, successful aging, and the attitude toward death, and the mediation effect of successful aging in the relationship between disease-coping level and attitude toward death. The survey process was directly collected through face-to-face interview method on 305 elderly people with chronic disease on August 15, 2012.
The collected data in this study were analyzed by using SPSS WIN 18.0 program. As a result of verifying X2(chi-square), the frequency analysis, t-test, and one-way ANOVA were carried out.
To examine the influence of the disease uncertainty upon disease coping, of the disease coping upon successful aging and attitude toward death, and of the successful aging upon attitude toward death, the multiple regression analysis was carried out. To examine the mediation effect of successful aging, the hierarchial regression analysis was carried out.
Summarizing the main findings, they are as follows.
First, the whole mean of the health status stood at 1.79 out of the 5-point perfection. Thus, the national meritorious persons recognized that their health status tends to be bad for the recent 1 month.
Second, the national meritorious persons, who were diagnosed diabetes from doctor, were the largest with 21.0%. The next was indicated to be cardiac disorder with 14.4%, high blood pressure and low blood pressure with 13.7%, arthritis with 10.3%, kidney ailment with 7.7%, and respiratory disease with 6.45%. The national meritorious persons, who were judged disability, accounted for 78.0%. The national meritorious persons in its opposite case accounted for 22.0%, thereby having been totalled to be not many.
The national meritorious persons, who were judged disability Level 6 from doctor, accounted for 39.1%, thereby having been the largest. The next was indicated to be in order of Level 5 with 23.9%, Level 7 with 23.5%, Level 3 with 8.0%, and Level 2 with 2.5%.
Third, the national meritorious persons, who use psychological and emotional service among welfare services of community, were the largest with 39.1%. The next was indicated to be in order of daily-life service with 35.5%, disease-related service with 18.2%, and health-promotion service with 9.3%.
Fourth, the whole mean in the uncertainty of disease accounted for 2.21% out of the 5-point perfection. Thus, the national meritorious persons showed low recognition on uncertainty of disease. The whole mean in the health promotion and disease-management behavior of national meritorious persons stood at 2.33 out of the 5-point perfection, thereby having been indicated to be low.
Fifth, as for the successful aging and the attitude toward death, the national meritorious persons were not so high in successful aging. The national meritorious person whose averagely monthly household income is under 1,500,000 won were higher in successful aging than other national meritorious persons. The whole mean in attitude toward death stood at 2.93. Thus, the national meritorious persons were indicated to be so positive in attitude toward death.
Sixthly, as for the influence in uncertainly of disease upon coping with disease, the national meritorious persons can be known that the higher uncertainty of disease leads to the higher daily-life performance ability. Uncertainty of disease(B=219, P人.001)
Seventh, as for the influence of the disease coping upon successful aging and attitude toward death, the national meritorious persons can be known that the higher self-control leads to the more negative in attitude toward death.
Eighth, as for the mediation effect of disease coping and successful aging, the national meritorious persons had the higher successful aging in the higher in uncertainty of disease, in health promotion, in disease management behavior, and in daily-life performance ability. It can be known that there is the mediation effect of the disease coping in the influence of uncertainty for disease upon successful aging.
It suggested a route, which leads to positive attitude toward death, by increasing the disease coping level and the successful aging in the elderly with chronic disease, thereby having proposed the practical intervention on disease, successful life, and death acceptance.
