옴(Scabies)은 옴진드기(Sarcoptes scabiei var. hominis)의 피부 기생으로 발생하는 감염성 피부질환으로, 확진의 근거로 사용되는 피부채취(Skin scraping)법의 낮은 민감도 및 다양한 임상양상으로 인하여 효율적인 진단에 어려움이 있는 실정이다. 본 연구에서는 기존 방법의 단점을 보완할 수 있는 높은 민감도와 특이도의 간편진단법을 확보하여 신속하고 정확한 진단에 활용하고자 하였다. 이를 위해, 옴진드기 특이적 염기서열인 Sarcoptes scabiei allergen Sar s 3(Sars3)와 Internal Transcribed Spacer 2(ITS2)를 전장유전체에서 발굴하고 이를 표적으로 하는 등온증폭법(loop-mediated isothermal amplification, LAMP) 기반 진단법을 개발하였다. 전통적인 DNA 추출법 대신 옴 의심 환부에서 확보한 각질 검체로부터 신속하게 주형 DNA를 확보할 수 있는 직접적인 용해 방법과 버퍼를 고안해 진단과정을 간소화하였다. 반응 조건은 LAMP에 활용되는 각 프라이머 세트별 증폭 속도와 특이성을 고려해 다양한 조합을 통해 최적화하였다. 그 결과 Sars3를 증폭하는 프라이머 사용 시 67℃, ITS2의 경우에는 61℃의 반응온도에서 35분간 증폭반응을 수행하는 것으로 최종 결정하였다. 해당 조건 하에서의 검출한도는 Sars3와 ITS2 서열을 증폭하는 프라이머로 반응 시 각각 1,000, 100개의 copy였으며, 이는 실제 진단 시 환자의 검체에서 발견되는 표적유전자의 수를 상회하여 안정적인 진단이 가능함을 시사한다. 또한 연구를 수행하며 진행된 300개 이상의 독립적인 음성대조군 반응에서 위양성은 전혀 관찰되지 않았으며 사람을 비롯하여 근연종인 집먼지진드기 유래 DNA에 대해서도 높은 특이도를 보였다. 옴 환자의 각질 검체를 활용하여 검증한 결과, 임상증상의 양상과 관계없이 옴 환자의 검체는 모두 양성으로 진단되었으나 비(非)옴 환자의 각질 검체에서는 반응이 나타나지 않아 성공적인 진단법 개발을 검증하였다. 추가적으로, 개발된 진단법의 현장 진단 적용 가능성을 평가하기 위하여 동결건조 형태의 반응물 시작품을 개발하고 안정성을 검증하였다. 또한 동티모르 보건부와의 협력연구를 통해 해당 시작품의 현장조사 시의 활용 가능성을 조사하였다. 본 연구에서 개발된 LAMP 기반 방식은 최소한의 휴대용 장비만으로도 옴 진단이 가능한 것으로 확인되어, 의료시설 및 자원이 한정된 현장 환경에서도 활용될 수 있는 플랫폼으로 기대된다.

Scabies, a common parasitic skin disease caused by Sarcoptes scabiei var. hominis, presents diagnostic challenges because of its diverse clinical manifestations and the limited sensitivity of traditional methods, such as skin scraping. Here, we developed a loop-mediated isothermal amplification (LAMP) assay targeting two S. scabiei-specific genes, Sarcoptes scabiei allergen Sar s 3 (Sars3) and Internal Transcribed Spacer 2 (ITS2). To enhance operational simplicity, a direct lysis protocol was employed, enabling rapid template DNA acquisition from skin scraping specimens without conventional DNA extraction. Reaction condition for each primer sets were optimized for amplification efficiency and specificity; reaction temperatures were determined to be 67℃ for Sars3 and 61℃ for ITS2. The assay achieved detection within 35 min, with limits of detection of 103 copies for Sars3 and 102 copies for ITS2. No false-positive signals were observed across the 144 negative control reactions for each target. The assay demonstrated high specificity for non-target DNA from humans and house dust mites. Clinical validation using skin scrapings from patients with classic and crusted scabies yielded consistently positive results, whereas samples from non-scabies controls yielded negative results. To further evaluate the potential for on-site diagnosis, lyophilized reagents were developed and validated through collaborative research with the Ministry of Health of Timor-Leste. The developed LAMP assay provides a diagnostic platform for scabies, requiring minimal and portable equipment, making it suitable for field or clinical settings, particularly in resource-limited areas.
