피하 종괴로 나타난 스파르가눔증 : 2예 보고 = Sparganosis presenting as a subcutaneous mass : case report

국문 초록 (Abstract)

스파르가눔증은 감염된 물벼룩이 들어 있는 물을 마시거나 개구리, 뱀 등이 생식했을 때 생기는 기생충성 감염으로 국내에서 많은 예가 보고 되어 왔다. 스파르가눔증은 흔히 피하조직을 침범하나 그 증상은 감염부위에 따라 다양하며, 유방에서 발견될 경우 만져지는 종괴로 내원하는 경우가 흔하다. 67세 여자환자와 53세 여자 환자가 각각 유방의 만져지는 압통이 없는 종괴로 내원하였으며, 검사 및 수술 결과 유방의 스파르가눔증으로 진단되었다. 스파르가눔증의 치료는 다른 기생충 질환과는 다르게 약물치료가 효과과 없으며, 외과적 절제만이 유일한 치료방법으로 알려져 있다. 본원에서 경험한 증례처럼 스파르가눔이 활발한 이동성을 보이는 경우가 있어 의심이 되는 경우 신속한 외과적 절제가 필요하다.

Rare snake or frog, infected water with a water flea or fresh water fish are usual important sources of sparganosis. In Korea, many cases have been reported. Sparganosis commonly involves subcutaneous tissue, but its clinical presentation is variable depending infected site.
Sparganosis in breast usually is presented with a palpable breast mass. 67-yearold and 53-year-old female visited hospital with non-tender palpable breast mass, they were diagnosed with sparganosis. Treatment is only surgical excision. The larva can migrate actively like our experiences, it is important that the mass should be excised immediately in case of suspicision.
