독일 수발보험에서의 수발수당에 대한 연구-쟁점과 효과평가- = A Study on Pflegegeld in German Pflegeversicherung－issues and effectiveness

국문 초록 (Abstract)

독일에서는 1970년대 말부터 인구고령화로 인한 장기요양욕구의 증대와 노인의료비용의 증가로 인한 재정압박 그리고 여성취업의 증가로 인한 사적 돌봄의 공백이 이슈화되면서 이에 대한 대응책으로서 수발보험을 도입하게 되었다. 수발보험에서 주목할 만한 것은 수발보험의 급여 중 하나인 수발수당이 현금급여라는 형태적 특성으로 인해 비공식서비스제공자에 대한 보상, 성역할 고착화, 소비자의 선택권 보장, 서비스의 질, 재정 등에서 다양한 효과를 낳고 있다는 점이다. 본 논문은 문헌연구를 통해 수발수당에 대해 주로 논의되는 쟁점 5가지를 중심으로 하여 그 효과를 평가해 보았다. 그 결과를 정리해 보면 수발수당은 비공식수발에 대한 보상의 측면에서는 어느 정도 긍정적이었으나 모순적이게도 성역할의 공고화를 강화하는 영향도 미치고 있었다. 그리고 수발수당은 서비스 수요자에게 서비스제공자에 대한 다양한 선택을 가능하게 하지만 이는 강요된 선택에 가깝고, 질 관리가 부족한 문제점이 드러났다. 마지막으로 수발재정에 있어서는 수발수당으로 인한 비용절감의 효과가 큰 것으로 평가되어 이 점은 장기요양체계의 구성에 있어서 다른 나라에 대한 긍정적 시사점이 될 수 있을 것으로 보였다.

다국어 초록 (Multilingual Abstract)

Long-term care has been rising on the policy agenda of many nations. The german government is worried about the increasing burdens of population ageing on health care system. In that context, taking on Pflegeversicherung as a new programme has appeared to be the best or efficient solution to issue of ageing society. Pflegegeld as cash payment has various effects for clients and family care-givers and cost containment of Pflegeversicherung. This paper has attempted to analyze the effects of Pflegegeld in 5 perspectives：1) payment for informal care-givers, 2) fixing traditional sex-role, 3) guarantee of service customer’s right of choice, 4) quality of service, 5) cost containment of Pflegeversicherung. Pflegegeld is positive evaluated in perspective of payment for informal care-givers and strength fixing traditional sex-role. Clients are able to choice various services by pflegegeld, but this choice is forcible demand and quality of informal service is lower than quality of formal service. Pflegegeld contributes to cost containment of service.
