    배경 : 식도암은 대부분의 환자가 고령이며 체중감소와 영양실조가 동반되어 있어 생존율이 낮은 수준임. 현재로서 수술적 절제가 완치를 목적으로 하는 최선의 치료방법으로 간주되고 있으나, 식도를 절제한 후 위장 또는 대장을 이용한 대용식도를 사용하기 때문에 수술 범위가 크며 영구적인 위장관 구조의 변화로 인하여 소화기계 전반에 걸친 위장관 증상을 호소하게 되며, 삶의 질에 큰 영향을 미침.
    목적 : 식도암 환자의 위장관 기능은 환자의 신체활동과 밀접한 관련이 있는 요인으로 특히 노인 암환자에서는 규칙적인 신체활동의 부족으로 신체활동이 저하될 수 있으므로 대부분의 환자가 고령인 식도암에서 신체활동은 더욱 중요함. 뿐만 아니라 신체 활동은 삶의 질에도 밀접한 관련이 있는 요인으로 나타남. 이에 본 연 구에서는 식도암 환자의 수술 후 신체 활동, 위장관 기능과 삶의 질의 변화를 살펴보고자 함.
    방법 : 2015년 3월 6일부터 2021년 10월 21일까지 삼성서울병원에 내원한 식도암 환자 중 총 335중 제외대상 40명을 제외한 총 295을 대상으로 연구를 진행함. 신체활동의 경우 걸음보수, METs-week, 6MWT, 악력으로 측정하며, 위장관 기능의 경우 GSRS, 삶의 질의 경우 EORTC QLQ-C30과 EORTC QLQ-OES18로 측정함. 선형혼합모형을 사용하여 반복하여 측정된 신체활동, 위장관 기
    능, 삶의 질 변화를 분석함.
    결과 : 대부분의 신체활동, 위장관 기능과 삶의 질은 수술 후 1개월과 2개월 시점 가장 낮으며, 수술 후 1년 시점까지 점차적으로 회복하지만 수술 전까지 완벽하게 회복하지는 않았음. 특히, 수술 후 1개월 시점 환자들은 연하 곤란, 입 마름, 변비, 복통, 소화불량, 설사, 역류 등의 증상을 경험하게 되며 삶의 질도 급격하게 감소하게 됨.
    결론 : 본 연구는 식도암 수술은 신체활동, 위장관 기능과 삶의 질은 서로 밀접한 연관이 있으며 수술 직후 많은 영향을 줄 수 있으므로 식도암 환자들을 위한 전문적인 수술 후 가이드라인이 필요성을 강조함.

    Purpose: Gastrointestinal function is closely related to physical activity, especially crucial for older esophageal cancer patients. Gastrointestinal function and low physical activity are also related to health-related quality of life. This study aims to explore how physical activity, gastrointestinal function, and health-related quality of life change after surgery.
    Methods: We examined 295 patients (excluding 40) out of 335 with esophageal cancer at Samsung Medical Center from March 6, 2015, to October 21, 2021. Physical activity was measured by daily step count, METs-week, 6-minute walk test, and grip strength. Gastrointestinal function was assessed with the Gastrointestinal Symptom Rating Scale (GSRS), and health-related quality of life was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 (EORTC QLQ-C30) and EORTC QLQ-OES18. Changes were analyzed with a linear mixed model.
    Results : Most physical activity, gastrointestinal function, and quality of life hit their lowest points at 1 and 2 months post-surgery but gradually improved up to 1 year after, not fully returning to pre-surgery levels. Notably, patients experienced symptoms like difficulty swallowing, dry mouth, constipation, abdominal pain, indigestion, diarrhea, and reflux at 1 month post-surgery, leading to a significant decline in health-related quality of life.
    Conclusion: This study investigates the need for postoperative guidelines for esophageal cancer patients. Recognizing how physical activity, gastrointestinal function, and life quality are connected is crucial for providing specialized care, especially after surgery.
