Ⅰ. Objective and Needs of R&D
1. Development of herbal medicines for dementia that overcome the limits of present dementia medicines and growth of dementia population
2. Development of herbal medicines for dementia that is based on the pattern identification of Oriental medicine and consider secondary symptoms of patients
3. Development of a herbal medicine is prepared by cooperation between the clinics of Oriental medicine, the oriental pharmaceutical department, the pharmaceutical department, CRO, the center for toxicity test and the pharmaceutical company
Ⅱ. Contents of R&D
1. Development of a protocol for the clinical research of dementia patients based on the Oriental medicine through the observation survey
2. Screening of herbal medicines from basic prescriptions, Hwangnyeonhaedok-tang and Sopungsungi-won
3. Effectiveness identification on the mechanisms of anti-oxidation, anti-inflammation, promotion of neuron differentiation and promotion of synapse revelation.
4. Medicine standardization, preparation formulation and safety confirmation.
5. Study of toxicity
Ⅲ. Results of R&D
1. We screened the candidating prescriptions through Ex-vivo tests and selected 6 herbs. We made up a preparation named K3 with the herbs. We proved its effectiveness for dementia through in vitro and in vivo tests and identified its mechanisms of anti-oxidation, anti-inflammation, promotion of neuron differentiation and promotion of synapse revelation. We published a paper named 'Effects of optimized-SopungSunkiwon on memory impairment and enhancement.' to 'Neuroscience Letters' with these results.
2. Development of a new dementia medicine that is not same with existing essence-tonifying and phlegm-dispelling formulas but a fever clearing formula to develop a medicine based on the theory of Oriental medicine : We selected Hwangnyeonhaedok-tang and Sopungsungi-won as basic prescriptions, sorted effect medicines according to the screening tests and added medicines that can help to protect brain nerves.
3. We proved the effectiveness of the extract of silk yarn(preparations of Bombyx Mori) and applied for a patent for a new composition comprising an extract of silk yarn. : Composition for preventing and treating neurological disease of brain or for the enhancement of memory comprising an extract of silk yarn.
4. Establishment a foundation for economically feasible medicine development by confirming an effectiveness of low-dose and low-cost medicine : We proved effectiveness of low-concentration herbal medicine and provided an opportunity to widen the ways of herbal medicine preparation development.
5. Development of a protocol for the clinical research based on oriental medicine considering symptoms : Oriental medicine clinicians select treatment prescriptions through a pattern identification based on the body symptoms. We suggest a possibility to develop a herbal medicine preparation that adjusted to the oriental medical clinical practice. We reported a result of research observing the clinical symptoms of dementia patients.
6. Although a high-dose sample showed a toxicity in toxicity tests, we applied for IND because the concentration was higher than effective range. We tested again with lower dose under the non-toxicity dose, confirmed the effectiveness and applied for IND.
Ⅳ. The Applicable Planning of results from R&D
1. The methodology of herbal medicine development based on the pattern identification and symptoms can be used to study the other diseases besides dementia.
2. It developed based on the evidences used by oriental medical doctors in clinical practice. So applicability of this dementia medicine seems to be high.
3. Suggesting a possibility to develop low-dose and low cost herbal medicines, it can be applied to develop an economically feasible herbal medicine.
4. The possibility of multi-purpose dementia drug development was suggested.
5. Multi-disciplinary drug development.

국문 초록 (Abstract)

Ⅰ. 연구개발의 목적 및 필요성
1. 치매 인구의 증가와 치매 치료제 개발의 한계를 극복한 한약 제제를 통한 치매치료제 개발
2. 한약물을 활용한 치매 치료제 개발에 있어서 한의학의 변증에 기반하고 치매 환자가 가지고 있는 2차적 증상을 고려한 약물의 개발
3. 한의학 임상, 한약학과, 약학과, CRO, 독성시험 기관, 제약회사 등의 협력으로 한약제제 개발
Ⅱ. 연구개발의 내용 및 범위
1. 치매환자에 대한 관찰조사를 통한 한의학 기반 치매 환자 임상시험 프로토콜 개발
2. 황련해독탕과 소풍순기원을 기본 약물로 하는 약물 선정
3. 항산화, 항염증, 신경세포 분화촉진 및 전후 시냅스 발현 촉진의 기전을 통하여 효능을 확인
4. 약물 표준화, 제제 제형, 안정성 확인

Ⅲ. 연구개발결과
1. 후보처방을 Ex-vivo실험으로 스크리닝하여 6가지한약물을 선별하여 K3를 도출하고, in vitro / in vivo연구를 통하여 항치매효능을 확인하였고, 그 기전으로 항산화, 항염증, 신경세포 분화촉진 및 전후 시냅스 발현 촉진을 확인함. Neuroscience Letters에 Effects of optimized-SopungSunkiwon on memory impairment and enhancement.라는 논문을 통하여 보고함.
2. 기존의 보정과 거담에서 벗어난 청열제로써 치매 치료제를 개발함으로서 한의학 이론에 기반한 약물 개발 : 황련해독탕과 소풍순기원을 기본 약물로 선정하고, 그 가운데 효능 있는 약물을 선별하고, 뇌신경보호에 도움을 줄 수 있는 약물을 추가하여 한약물을 활용한 치매 치료제 개발을 시도함
3. 잠사의 효능을 검증하였고, 잠사를 포함한 새로운 조성물로 특허를 출원하였음 : 잠사 추출물을 포함하는 뇌신경질환 예방 및 치료용 또는 기억력 증진용 조성물
4. 저농도와 저비용 약물의 효능을 입증함으로서 경제성 있는 약물 개발의 기반을 마련 : 기존의 고농도 한약물의 농축을 통해 개발되던 답습에서 벗어나, 저농도에서도 효과가 있음을 밝혀, 한약제제 개발의 폭을 넓힐 수 있는 계기를 마련함.
5. 증상을 고려한 한의학에 기반한 임상시험 프로토콜 개발 : 치매 환자의 40% 이상에서 인지장애와 함께 열증을 동반한 2차 증상을 가지고 있으며, 한의학의 특성상 신체 증상에 기반한 변증을 통하여 치료 처방을 선정하므로, 한의학 임상에서의 활용에 맞춘 한약제제 개발의 가능성을 제시, 동의신경정신과학회지에 치매 환자에 대한 관찰 -치매 환자의 임상증상에 대한 조사 연구를 보고함
6. 독성연구를 통하여 고용량에서 독성이 발현되었으나, 효력용량범위를 넘어서는 농도여서 IND신청을 진행하였으나, 식약청 면담에서 신조성물의 경우 안전계수 10을 적용하여 무독성용량을 인간용량 환산시 1/10으로 줄여야 한다고 하여 효력용량범위를 벗어나, 안전계수를 고려한 무독성용량 이하의 저용량으로 추가 효력 실험을 진행하여 효력이 입증되어 IND를 신청함.
Ⅳ. 연구개발결과의 활용계획
1. 변증과 증상에 기반한 한약물 개발의 방법론을 바탕으로 치매뿐 아니라 타 질환 연구의 기틀로 활용
2. 한의사들이 실제 임상에서 활용할 수 있는 근거를 바탕으로 개발이 진행되어, 치매 치료제가 개발된 후 한의사들이 실제 활용도가 높을 것으로 보임.
3. 저용량과 저비용의 한약물 개발 가능성을 제시하여, 경제성 있는 한약물의 개발에 활용.
4. 치매 유발 가설에 대한 다양한 가설에 맞춘 모델을 적용한 연구를 진행하고 이를 기반으로 기전을 연구하여 치매연구기초자료를 제공하고, 다목적 치매 치료제 개발의 가능성을 제시함.
5. 한의학 임상, 한약학, 약학, CRO, 제약회사, 독성시험 기관과의 유기적 연구를 통한 다학제간 연구 모델을 제시함.
