본 연구의 목적은 자해·자살 시도 경험이 있는 청소년의 위기는 어떤 원인에 의해 유발되어 어떤 과정을 거쳐 진화하는지, 그리고 자해·자살 시도 이후 어떤 조건과 상호작용을 거쳐 삶을 다시 이어가게 되는지를 탐구하는 것이다. 이를 위해 가톨릭청소년이동쉼터 ‘서울아지트’에서 상담과 관리를 받은 만 13~24세 고위기 청소년 14명을 연구참여자로 선정하여 반구조화 심층면담을 실시하였다. 수집된 면담자료는 체계적 근거이론 절차에 따라 개방코딩, 축코딩, 선택코딩을 거쳐 분석하였다.
분석 결과, 고위기 청소년의 위기 경험은 벗어나고 싶은 가족, 무너져버린 내면 상태, 지속적인 관계 손상이라는 세 가지 인과적 조건을 토대로 형성되었다. 이러한 조건은 함께 살아도 혼자인 가족, 자해와 자살로 향하는 마음과 환경, 친구·어른·제도 모두 보호막이 되지 못함, 어려운 경제적 환경과 생존을 위한 위태로운 선택으로 구성된 네 가지 맥락적 조건 속에서 유지되고 심화되었으며, 그 과정에서 청소
년들은 관계적 단절 속에서 고통을 조절하고 존재를 확인하려는 시도를 중심현상으로 경험하였다. 중심현상은 죽고 싶다는 생각과 자해의 동기에서 출발해 자해 시도의 시작과 이후의 양가적 감정, 자해 행동의 심화와 자살 충동으로의 발전, 그리고 누군가 알아봐주길 바라며 이루어지는 자살 시도로 전개되었다.
한편 위기 흐름이 완화되거나 전환되는 국면에서는 관계적 안전과 선택 가능성을 조절하는 중재적 조건이 작동하였고, 이에 따라 참여자들은 위험한 고통 조절 방식에 머무르기보다 관계를 통한 조절 전략으로 전환하고 삶을 재조정하는 작용 및 상호작용을 수행하였다. 그 결과는 단기(자해와 자살 행동의 일시적 중단과 재위기), 중기(가족관계 및 생활 기반의 재구성), 장기(목표 설정과 내적 자원 축적을 통한 주체성 회복)의 시간적 위계에 따라 다양한 변화 궤적으로 나타났다.
이들의 위기 회복과정은 위기 행동의 완전한 소거가 아니라, 위기 국면이 재현 되더라도 회복의 경로로 되돌아올 수 있는 조정 능력이 축적되는 과정으로 이해되며, 재위기와 되돌아감, 재진입 가능성을 포함하는 비선형적 궤도로 확인되었다. 위기 국면에서도 관계가 끊어지지 않았다는 경험은 중재적 조건들을 하나로 묶어 주는 핵심으로 작동했으며, 본 연구는 이를 ‘비빌언덕’으로 명명하였다. 본 연구는 이러한과정모형과 핵심 메커니즘을 통해 고위기 청소년을 단순한 도움의 대상이 아니라 위기 경험 속에서 학습하고 변화하는 주체로 조망하며, 현장에서 개입 시기와 지원 방식을 조정하는 데 활용 가능한 실천적 근거를 제공하는 데 의의가 있다.

This study explored, from a process perspective, how high-risk adolescents with histories of self-harm, suicide attempts, or both understand the background and contextual conditions under which their crises are formed and intensified, and how they continue their lives following self-harm and/or suicide attempts through particular conditions and interactions. Fourteen high-risk adolescents aged 13–24 who had received case management and counseling services at the Catholic outreach youth shelter Seoul Ajit participated in the study. Data were collected through semi-structured, in-depth interviews and analyzed using the systematic grounded theory procedures of open coding, axial coding, selective coding.
Findings indicated that crisis experiences were formed on the basis of causal conditions comprising (a) a family one wants to escape, (b) a collapsed internal state, and (c) enduring relational damage. These conditions were maintained and exacerbated within contextual conditions consisting of (a) feeling alone despite living with family, (b) psychological states and environments that draw adolescents toward self-harm and suicide, (c) the absence of protective buffers from peers, adults, and institutions, and (d) adverse economic circumstances and precarious survival-oriented choices. Within this context, participants experienced the central phenomenon of attempting to regulate pain and affirm their existence amid relational disconnection. This phenomenon unfolded from suicidal ideation and motivations for self-harm, to the onset of self-harm attempts and ensuing ambivalent emotions, to the escalation of self-harm and the development of suicidal impulses, and ultimately to suicide attempts enacted with the hope that someone would notice and interven.
When the crisis trajectory began to ease or shift, intervening conditions that regulated relational safety and the perceived availability of alternatives became salient. Accordingly, rather than remaining in high-risk pain-regulation patterns, participants engaged in action/interaction strategies characterized by a transition toward relationship-based regulation and by life recalibration. Outcomes emerged along a temporal hierarchy, ranging from short-term changes (temporary cessation of self-harm and suicidal behaviors alongside recurrent crises), to mid-term changes (reconstructing family relationships and stabilizing living foundations), to long-term changes (restoring agency through goal-setting and the accumulation of internal resources), yielding diverse trajectories of change.
In this study, crisis recovery did not signify the complete elimination of self-harm or suicidal behaviors; instead, it was conceptualized as an accumulative process in which adolescents develop the regulatory capacity to return to recovery pathways even when crisis episodes re-emerge. The recovery process followed a non-linear trajectory that included recurrent crises, regression, and re-entry. A key finding was that the experience of maintaining relational ties during the crisis phase served as an integrative core condition among the intervening conditions and was conceptualized as a relational anchor(Bibilleondeok). This study offers a process model and core mechanisms that reframe high-risk adolescents not merely as recipients of help but as agents who learn and change within crisis experiences, providing practice-relevant evidence to inform the timing and tailoring of field-based interventions.
