질병예측성 유전자검사 관리방안 연구 = Study for managing system of genetic testings that are intended to predict diseases

국문 초록 (Abstract)

최근 유전학 기술의 비약적 향상으로 시행가능한 질병예측성 유전자 검사는 급증하고 있다. 그러나 질병예측성 유전자 검사 중 상당수가 검사의 정확도 및 임상적 유용성이 입증되지 않은 경우가 많다. 검사의 정확도 및 임상적 유용성이 부족한 유전자검사 결과는 수검자로 하여금 부적절한 투약, 생활습관의 변화, 정신적 혼란 등을 야기할 가능성이 높다. 이러한 이유로, 본 연구자들은 질병예측성 유전자검사를 관리방안에 대하여 연구를 진행하였다. 본 연구를 위해 국내외 질병예측성 유전자 검사의 현황을 조사, 분석하였으며, 이에 관련한 국내외 제도를 조사하였다. 또한 의견 수렴을 위해 자문회의, 공청회, 설문조사를 시행하였다. 결론적으로, 본 연구자들은 질병예측성 검사의 현실적인 관리를 위해서 유전자 항목별 검토 보다는 유전자 특성에 따른 카테고리로 분류하여 관리하는 것(class I부터 VI까지)을 제안하는 바이다. 이러한 분류과정에는 국내의 유전자검사와 관련한 법, 임상적 유용성, 및 검사 목적 등이 반영되어야 한다. 이를 위해, 본 연구자들은 유전자검사 신고서식과 유전자검사의뢰서를 수정하였다. 이와 더불어 이러한 유전자검사를 평가하기 위해 상시적인 전문가위원회 활성화를 위한 제도적 확립을 제안하고자 한다. 또한, 각 분류별 구체적인 평가 기준은 이후 연구에서 확립이 되어야 할 것이다.

다국어 초록 (Multilingual Abstract)

In recent years, as more and more predictive genetic testings have become available because of the rapid improvement of genetic technologies. However, a relatively larger number of predictive genetic testings often lack certainty and clinical utility. They are misleading or misguiding. In addition, overstatement of effectiveness or minimization of risk has the probability of leading to inappropriate changes in medication, life style, or confusion, etc.. In the present study, we represent the proposed approach of the managing system for predictive genetic testing. We analyzed domestic and foreign current status of predictive genetic testings and investigated the genetic testing related systems. For gathering the opinions of specialists or practicians, we held advisory conferences and a public hearing. In addition, we conducted a survey. For the practical managing of predictive genetic testings, we have proposed the classification (from class I to VI) of the current predictive genetic testings. This classification should reflect domestic law, appropriately clinical utility, and informations about the purpose. For this, we have proposed the revision of registration form of new genetic testings and requisition form of genetic testing. In addition, we have proposed to establish the system for invigorating a continuous expert panel for evaluation of these genetic testings. The concrete valuation basis of each categorization should be established through the further research.
