경추 전방 유합술에서의 자가 양면 피질골 이식의 유용성|RISS 상세보기

Study Design: Retrospective study.
Objectives: To demonstrate the fusion rate, degree of subsidence and donor site morbidity of anterior cervical interbody fusion with autogenous bicortical iliac bone graft and anterior cervical locking plate.
Summary of Literature Review: In anterior cervical discectomy and fusion with autogenous tricortical iliac bone graft, a large percentage of patients report chronic donor site pain.
Materials and Methods: Retrospective research was done for 39 patients who underwent anterior cervical interbody fusion with autogenous bicortical iliac bone graft, from January 2006 to July 2011, with a follow up period of longer than 1 year. Fusion rates and subsidece of the graft is estimated with radiographs. Neck pain and donor site pain was estimated with visual analogue scale (VAS) and dysfunction was estimated with the neck disability index (NDI).
Results: A 95% of patients who underwent anterior cervical interbody fusion with autogenous bicortical iliac bone graft revealed definitive fusion with little amount of subsidence. The mean VAS score was 0.7 on the donor site and the mean NDI score was 3.8 at the final visit. There was excellent clinical outcome without complication at the donor site or the recipient site.
Conclusions: Anterior cervical interbody fusion with autogenous bicortical iliac bone graft showed high fusion rates and minimal subsidence with excellent clinical outcomes. Therefore, bicortical iliac bone graft is an effective operational procedure in anterior cervical interbody fusion.

국문 초록 (Abstract)

연구 계획: 후향적 연구목적: 자가 양면 피질골 이식과 경추 전방 잠김 금속판을 이용한 경추의 전방 추체 유합술에서 유합률, 골침하 정도, 공여부의 이환에 대해 알아보고자한다.
선행문헌의 요약: 자가 삼면 피질 장골 이식을 이용한 경추 추간판 절제술 및 전방 추체 유합술 후 많은 환자들이 공여부의 만성 통증을 호소하였다.
대상 및 방법: 2006년 1월부터 2011년 7월까지 본원에서 장골에서 채취한 자가 양면 피질골 이식을 이용하여 경추 전방 추체 유합술을 받고 1년 이상 추시가 가능했던 39명의 환자에 대해 후향적 연구를 시행하였다. 방사선학적 사진을 이용하여 유합률, 골침하 정도를 조사하고, visual analogue scale(VAS)을 이용하여 경부, 공여부 통증을 평가하고 neck disability index(NDI)를 이용하여 기능 장애를 평가하였다.
결과: 경추 전방 유합술에서 자가 양면 피질골 이식을 이용한 결과, 95%의 환자에서 확실한 골유합을 얻었으며 및 골침하 정도도 경미하였다. 최종 추시시에 VAS를 이용하여 평가한 공여부 통증은 0.7점이었고, NDI를 이용하여 평가한 기능 장애는 3.8점이었다. 그 외 이식부나 공여부의 다른 합병증도발생하지 않는 우수한 임상 결과를 보였다.
결론: 자가 양면 피질골 이식을 이용하여 경추 전방 유합술을 시행한 결과, 우수한 유합률과 경미한 골침하 정도를 보였으며, 임상 결과도 매우 우수하였다. 따라서 경추 전방 추체 유합술에서 양면 피질골 이식이 유용할 것으로 생각된다.
