The 7S collagen is a large collageneous fragment of basement membrane, which was recent- ly reported to have increased activity in various liver diseases. In order to deterrnine whether 7S collagen concentrations of serum and ascites is helpful to make diagnosis of various liver diseases, we measured 7S collagen concentrations of serum and ascites in various liver diseas- es. Normal values obtained from 20 healthy controls were 3.12k0.70ng/ml for 7S collagen concentration in the serum. Serum concentrations of the 7S collagen exceeding 4.52ng/ml (Mean+2SD) was defined as positive. The mean values of 7S collagen concentration of serum were 5.65k3.20ng/ml in chronic hepatitis, 5.21+4.67ng/rnl in acute hepatitis, 12.54k5.45ng/ ml in liver cirrhosis, and 10.05k4.72ng/ml in hepatoma. The positive rate was extremely high in hepatoma (100%) and liver cirrhosis(97.1%) but low in chronic hepatitis(59.9%) and acute hepatitis(33.3%). The mean values of 7S collagen concentration in ascites were 59.05+ 18.20ng/ml in liver cirrhosis and 55.32k4.23ng/ml in hepatoma. The 7S collagen concentra- tion of ascites shows no parallel correlation to that of serum in patients with ascites.(Korean J Gastroenterol 1994; 26: 655 661)
