건강과 복지 : 권리와 사회정의의 관점에서 = Health and Healthcare in Welfare State: Social Justice as a New Principle

Explosion of healthcare expenditure will politicize issues of health financing and resource allocation which has been understood as technical policy agenda. Political fundamentals of health and healthcare system will be threatened by emerging problems of rapid aging, technology innovation, and new family structure. However, current health-related social policies are to cope with traditional social risks, labor market related ones in particular. Also 'transformation' pf welfare state, such as social investment state, is not adequately dealing with health and healthcare by mainly focusing on financial sustainability. This paper is to insist health and healthcare system including health financing must be radically reformed, based on a new principle of equity and social justice in health and healthcare.

국문 초록 (Abstract)

한국에서도 나타날 의료비 폭증은 그 동안 기술과 관리의 차원이던 건강과 보건의료분야의 자원조달과 배분의 문제를 정치화·사회화 할 것으로 예상된다. 고령화와 생산인구의 감소, 생명연장 기술의 발전, 새로운 가족관계 등은 기존의 건강과 보건의료체계를 근본에서 위협할 것이며, 기술적인 접근만으로 이러한 도전에 대응하기 어렵다. 이러한 압력은 건강과 보건의료, 그 중에서도 건강보장의 구조에 대전환을 촉진할 공산이 크다. 사회적 제도로서의 건강보장이 복지의 한 요소로서 전통적인 사회적 위험에 대응하는 것이 목적이었다면, 새로운 건강과 보건체계가 당면해야 하는 상황은 과거의 사회적 위험, 특히 노동과 관련된 위험이라는 전통적 범주를 넘어설 수 있다. 또한 보건의료비 지출의 큰 비중에서 알 수 있듯이 사회투자국가론 등 새로운 복지국가의 지향성만으로는 해결할 수 없는 과제를 제기한다.
이상과 같은 맥락에서 이 논문에서는 변화하는 복지국가의 구조 안에서 건강보장의 범위를 넘어 건강과 보건의료의 과제를 설명하고 행동의 지침이 될 일관된 원리를 모색하고자 한다. 새로운 복지국가의 구성원리는 재정 측면에서의 지속가능성을 넘어 건강과 보건의료의 불평등을 줄이고 그 배분과 분포가 사회정의에 부합하여야 한다는 것이다. 현재로서는 이러한 원리가 규범적 차원을 넘기 어렵겠지만, 정의에 기초하여 새로운 원리를 모색하는 것은 복지국가의 새로운 구성과 이에 기초한 새로운 사회적 행동의 원칙을 수립하는 데에 중요한 기여를 할 것이다.
