초기 백혈구감소증으로 나타난 만성호중구백혈병 증례 보고 = Chronic Neutrophilic Leukemia with Initial Leukopenia: A Case Report and Review of the Literature

국문 초록 (Abstract)

만성호중구백혈병(CNL) 은 골수증식성질환 (MPN)의 하나로 성숙 한 중성구 증가증이 지속되는 것을 특징으로 한다. 초기에 백혈구 감소증과 빈혈을 보였던 52세 환자가 추후 만성호중구백혈병으로 진단된 증례를 보고하고자 한다. 스테로이드 투여 후 중성구 수가9 14.260×10 /L로 급증하였다. 시행한 검사에서BCR/ABL, JAK2, , 및 FGFR1, PDGFRA, PDGFRB 이상은 발견되지 않았 MPL CALR 으며 CSF3R 유전자 검사에서 p.T618I 돌연변이가 확인되어 만성호 중구백혈병으로 진단되었다. Hydroxyurea 투여를 시도하였으나 효과는 단기간만 지속되었다. 본 증례는 내원시 백혈구감소증을 보인 환자이더라도 중성구 수치가 크게 증가할 경우 CNL을 의심 할 필요가 있음을 시사한다.

Chronic neutrophilic leukemia (CNL) is a myeloproliferative neoplasm (MPN) characterized by persistent mature neutrophilia. We report a case of a 52-year-old man with CNL who initially presented with leukopenia and anemia. His neutrophil count increased to 14.26×10 /L following steroid 9 treatment. Tests for JAK2 , MPL , and CALR mutations, as well as FGFR1 , PDGFRA , and PDGFRB rearrangements, were negative. Polymerase chain reaction (PCR) sequencing of the CSF3R gene revealed a p.T618I mutation. Treatment with hydroxyurea demonstrated only a short-term response. This case highlights the importance of considering CNL in patients presenting with severe neutrophilia, even if leukopenia was observed at the initial visit.
