Background and Purpose : Korea is expected to become super-aged society in the near future and face great burden due to health problems of elderly people. Long-term prospective cohort studies are required to develop effective strategies for the prevention and management of major health problems among elderly Koreans. The purposes of this project were to design a new elderly cohort study and to develop a protocol and an operational plan for the cohort study.
Contents and Methods : The project contents include 1) literature review on previous observational studies on health problems of elderly, 2) selecting major outcomes of interest based-on morbidity and mortality data, 3) design of a cohort study to identify major risk factors for the major outcomes, and 4) suggestion of data management and sharing policies. To complete these tasks, we reviewed methodologies which were used for existing elderly cohort studies, analyzed data on mortality and medical service utilization of Korean elderly and future-elderly, benchmarked outstanding elderly cohort studies in USA, and obtained expert opinions.
Results : We selected four key health-related problems among elderly, including cardiovascular disease, cognitive function, mobility, and social support. Study hypotheses were generated to assess interactions between the four health problems and their impact on healthy lifespan and quality of life. Based-on the study hypotheses, we designed a new elderly cohort consisting of 5,000 people aged 65 years of older. Around two research centers are expected to perform enrollment, screening, baseline and follow-up examinations. Total study period is 10 years for a baseline examination and two sets of follow-up examinations. Estimated study budget ranges between 3,007 million KW and 4,004 million KW for enrollment and baseline examination at two research centers over 4 years. This project report also includes not only core study design (specified objectives & hypotheses, health problems of interest, inclusion & exclusion criteria, required sample size, yearly plan of baseline & follow-up examinations, and estimated budget) but also variable lists, measurement protocols, and other documents for baseline examination.
Expected Contributions : In the short term, this report will serve as core evidence for launching a new elderly cohort project by the Korean CDC, and as a guideline for cohort study investigators. This project will contribute to the successful beginning and performance of a new elderly cohort study. In the long term, the new elderly cohort will provide important new knowledge on the determining factors of healthy lifespan and quality of life, and contribute to the development of efficient disease prevention and management strategies.
