Clostridium difficile 독소 A 검출을 위한 효소면역법의 비교 = Comparison of Two Enzyme Immunoassays for Clostridium difficile Toxin A

국문 초록 (Abstract)

서론 : Clostridium difficile(C. difficile)은 병원 감염을 일으키는 원인균 중의 하나로 독소 생성 여부에 관한 정확한 진단이중요하다. C. difficile배양 및 세포독성검사 대신 일반적으로 용이하게 사용되는 독소 A 효소면역법 중 ELFA법과 ELISA법을C. difficile배 양 및 독 소 A P C R 와 상 호 비 교 하 여 진 단 적 의를 평가하고자하였다.재 료 및 방 법 : C. diffcile배양 검사가 의뢰되었던 65예의 설사변을 대상으로 혐기성 배양을 시행하였으며, 같은 검체로 ELFA(VIDAS C. difficileToxin A II assay, France)법과 ELISA법(C.DIFFICILE TOX A II, TECHLAB, USA)을 이용한 독소A 검사를 시행하였다. 독소 A PCR 검사는 배양에서 분리된 C.difficile23균주를 대상으로 시행하였다.결과 : 독소 A 효소면역검사간의 결과 일치율은 76.9% (50/65)였다. 배양 일치율은 VIDAS 83.1% (54/65), TECHLABC. difficile독소 A 효소면역법의 비교 41164.6% (42/65)였다. 독소 A PCR 결과는 양성 13예(56.5%),변이주 6예(26.1%), 음성 4예(17.4%)였다. PCR 및 배양 기준의 VIDAS 독소 A 검사의 민감도는 84.6% (11/13), 특이도는98.1% (51/52)였고 TECHLAB법은 민감도는 84.6% (11/13),특이도는 67.3% (35/52)였다. 양성 및 음성 예측률은 VIDAS91% (11/12)/96.2% (51/53), TECHLAB 78% (11/14)/94.6%(35/37)이었다. 독소 B PCR은 독소 A 양성, 변이주, 음성 균주의 100%, 83.3% 및 50%에서 양성으로 나타났다.
