병적 도박에 대한 이중초점 도식치료 적용 가능성의 탐색 = An Exploration of Application to Dual Focus Schema Therapy for Pathological Gambling

The characteristics of pathological gambling(PG) are similar to those of substance dependence symptom in terms of the process of addiction and relapse. Relapses of PG, which complicate its treatment, arguably is a result of automatic thoughts and behaviors. Dual Focus Schema Therapy (DFST) comprises the conventional techniques of Relapse-Prevention Coping Skills, which focuses on symptoms, the Early Maladaptive Schema (EMS) and schema-focused techniques on coping style. DFST, which consists of evaluation and conceptualization of the EMS and the coping style, is a time-limited and procedural treatment model. This paper examines the similarities between symptoms of PG and substance dependence and investigates if Ball and Young's DFST that was applied to substance dependence patients (2000), also is applicable to pathological gamblers.

국문 초록 (Abstract)

병적 도박은 외부 물질의 사용으로 인한 중독인 물질사용장애와는 달리 개인의 강박적인 행동의 반복으로 인해 중독이 되지만, 그 진행 과정, 내성, 금단, 재발 등의 측면에서는 물질의존 증상과 동일하다는 특징을 보인다. 도박행동과 같이 표면적 행동이 심리적 이유와 역기능적으로 연결된 경우, 표면행동에 영향을 주는 심층적인 문제를 이해하고 이를 변화시킬 수 있는 인지행동치료가 적합하다. 한편, 도박치료를 어렵게 하는 재발은 자동적 사고와 행동의 결과로 볼 수 있다. 이중초점 도식치료(Dual Focus Schema Therapy: DFST)는 전통적인 증상 중심의 재발방지 대처기술 기법들(Kadden et al., 1992; Marlatt & Gordon, 1985; Monti et al., 1989)과 초기부적응도식, 그리고 대처 방식에 대한 도식중심적 기법들로 이루어진 개인치료모델이다. 본 논문에서는 병적 도박자들이 보이는 증상 특성들과 물질의존 증상의 유사한 부분들을 살펴보고 Ball과 Young(2000)이 물질의존증상에 사용했던 DFST를 소개하여 이를 병적 도박자들에게 적용할 수 있는 가능성을 알아보고자 하였다. 그리고 이것을 병적 도박자들에게 적용하게 될 때 수정해야 할 부분에 대해 언급하였다.
