수술로 치료한 소화성 궤양의 시대적 분석 = Trends in the surgically treated peptic ulcer

To find out chronological changes of the peptic ulcers treated surgically and late complications according to the operative procedures 512 patients were analysed who received operation for peptic ulcer at the department surgery. Wonkwang university school of medicine, from January 1983 to December 1994. Analysis was made for every four year basis, and late complications of 127 patients who could be followed up were reviewed.
The results were as follows:
(1) Peptic ulcer operations decreased as 201cases, 158cases, and 153 cases in period 1
(1983-1986), 2 (1987-1990), and 3 (1991-1994).
(2) Incidence of gastric ulcer in peptic ulcer decreased from 22.4% in period 1 to 16.3% in period 3.
(3) Incidence of operation for hemorrhage decreased gradually from 60% to 32% in gastric ulcer and from 6.4% to 1.6% in duodenal ulcer
(4) There were no chronogical changes in the sex ratio, mean age, symptom duration, rate of the mtraperitoneal free air in perforation cases, mortality, and postoperative early complications.
(5) The incidence of the gastric ulcer increased from 11.4% to 40% in the winter. while
the incidence of duodenal ulcer decreased from 39.7% to 17.2% in the autumn.
(6) There were little chronological changes in the operative procedures for gastric ulcer, while those for duodenal ulcer had changed from the truncal vagotomy with antrectomy or hemigastrectomy to the truncal vagotomy with drainage procedure.
(7) The late complication rates were the highest in the truncal vagotomy with antrectomy or hemigastrectomy (10.2%).
