분노조절 장애자의 부정정서 특성연구를 통한 진단지표 개발

국문 초록 (Abstract)

분노 조절 장애자들의 부정 정서 처리과 관련된 심리 연구들은 다수 존재함에도 불구하고 그들의 부정 정서 처리에 기저한 뇌 기능 연구는 제한적이다. 정확한 진단과 효과적인 치료를 위해서는 명확한 병인 기전 규명이 선행되어야 하는데, 분노 조절 장애자의 정서 처리 기전을 과정별(정서 지각, 경험, 조절)로 구분한 연구는 거의 전무하며 이들의 정서 처리 기전에 관련된 뇌기능에 관해서는 알려진 바가 없다. 따라서 본 연구는 분노 조절 장애자의 정서 지각, 정서 경험, 정서 조절과 관련된 심리적 그리고 뇌 기능적 특성을 규명하고자 하였다. 부정 정서 처리와 관련된 뇌 반응 특성을 규명하기 위하여 자기 공명 영상 기법(fMRI)를 이용하였다. 정서 지각과 정서 경험, 정서 조절을 담당하는 신경 시스템은 구분되어 있어, 이 세 과정을 달리 구분하여 실험과제를 달리 제시한 결과, 이 세 과정 모두에서 분노 조절 장애자가 정상인에 비해 기능적 손상을 보였으며, 이와 관련한 뇌 기능의 결함 즉 양측 편도체와 좌측 미상핵의 활성화 이상이 밝혀졌다. 또한 본 연구는 부정 정서 처리 특성에 따른 뇌 반응 특성과 분노 조절 장애자의 심리 특성을 준거로 하여 분노 조절 장애의 통합 진단 지표를 발견하였고, 그 결과 약 80%의 정확률을 가진 분노조절장애 진단 모형을 개발하였다.

Despite of a number of psychological studies related to the negative emotional processing among the people with anger regulation disorder, the studies of neuroimaging data based on their negative emotional processing are limited.
To classify the diagnosis and develop effective treatment, the pathogenesis should be preceded clearly. However, there is almost no study to identify emotional processing mechanism, including emotion perception, experience and regulation, among the people with anger regulation disorder. Therefore, this study aims to investigate psychological and brain functional characteristics related to emotion perception, experience, and regulation among the people with anger regulation disorder. In the results of neuroimaging analysis of three emotional tasks, the people with anger regulation disorder showed behavioral impairment as compared with the normal group in all three stages, as the emotion perception, emotional experience, and they showed the functional impairment in the various area such as the bilateral amygdala and the left caudate. We also found an integrated diagnostic indexes of anger regulation disorder based on brain response and psychological characteristics.
