    Abstract

    Patient safety nursing activities of nurses in convalescent hospital according to patient safety culture recognition, patient safety knowledge and incident reporting attitude.

    Research Purpose: The purpose of this study was a descriptive research study conducted to provide basic data for the development of educational programs related to patient safety nursing activities for nurses in convalescent hospitals by analyzing the difference in the level of patient safety nursing activities according to patient safety culture recognition, patient safety knowledge and Incident reporting attitude.

    Research Method: The subjects of this study were 188 nurses working at the convalescent hospital in C city G province. The criteria for selection of the subjects were nurses who had at least 3 months' work experience at the current convalescent hospital and who voluntarily agreed to participate in the study. The data collection period was from July 1, 2018 to July 30, 2018, and the researcher provided the explanation of the questionnaire directly and received the written consent, and then collected the data using the self-report questionnaire method. The questionnaires survey were conducted on 198 nurses. After excluding the questionnaires which were not answered and unanswered questionnaires, a total of 188 data were analyzed using the SPSS 23.0 program. The results were as follows.

    Research Results: The data are
    1. The characteristics of the subjects in this study were as follows.
    Out of the 188 total, 176(93.6%) were female nurses and 12(6.4%) were male nurses. Most of them were female nurses. The average age was 40.05 years old and married was 126(67.0%). Of the nurses who graduated from college, 127(67.6%) were the majority, and 57(30.3%) nurses graduated from four-year undergraduate program and 4(2.1%) nurses graduated from graduate school. The average working experience was 11 years and 10 months, and the average career at convalescent hospital was 3 years and 4 months. The number of general nurses was the highest with 146(77.6%), followed by 82(43.6%) nurses with 3 shifts, 27(14.4%) with day/evening shift and 19(10.1%) in night dedicated. More than 90% of the subjects had experience of receiving patient safety education (181 persons, 96.3%) and participated in the evaluating hospital certification (176 persons, 93.6%).

    2. The recognition score for patient safety culture of the subjects was 3.84 out of 5, knowledge of patient safety was 3.82 out of 5, and attitude score of incident reporting was 3.70 out of 5. In the recognition by sub-area for organizational dimension among the patient safety culture recognition, the score of leadership was 4.03, patient safety policy and procedure 3.78, patient safety improvement system 3.64, and the recognition score of the organization's leadership was the highest and the recognition score of the organization's patient safety improvement system was the lowest. In the recognition by sub-area for department dimension, the score of teamwork was 3.92, non-punitive environment 3.69. In the recognition by sub-area for individual dimension, the score of patient safety knowledge and attitude was 4.03, patient safety priority 3.41, with low recognition of patient safety priority. In the sub-area for incident reporting attitude, the score of concern about evaluation use was 3.41, beliefs about improvement effect 3.91, reporting intention 4.00, and the attitude toward reporting intention was high. The performance score for patient safety nursing activities of the subjects were generally high, with 4.65 out of 5.

    3. The patient safety culture recognition showed significant differences according to educational background of the subjects, and patient safety knowledge showed significant difference according to age, working experience, current position, working style and experience of certification assessment in hospital.
    There was a significant difference in the incident reporting attitude according to the subjects' religion, working experience, career experience at convalescent hospital, position, working style, and patient safety nursing activities were found to be significantly different according to age, marital status, education level, career experience, and current position.

    4. Patient safety nursing activities were positively correlated with patient safety culture recognition(r=.36, p<.001), and also showed a significant positive correlation with patient safety knowledge(r=.48, p<.001) and incident reporting attitude(r=.31, p<.001) at correlation between patient safety nursing activities and patient safety culture recognition, patient safety knowledge, incident reporting attitude.

    5. Patient safety nursing activities were significantly different between the subjects who had a high recognition of patient safety culture(4.75±.26) and those who were low(4.54±.48), and patient safety nursing activities are found to be highly performed compared to low-level groups with high recognition of patient safety culture(t=-3.75, p<.001).
    Patient safety nursing activities were significantly different between the subjects who had a high knowledge of patient safety(4.79±.22) and those who were low(4.46±.49), and patient safety nursing activities are found to be highly performed compared to low-level groups with high knowledge of patient safety(t=-5.72, p<.001).
    Patient safety nursing activities were significantly different between the subjects who had a high group(4.73±.34) of incident reporting attitude and those who were low(4.53±.45), and patient safety nursing activities are found to be highly performed compared to low-level groups with high group of incident reporting attitude(t=-3.40, p=.001).

    Conclusion: Patient safety nursing activities were positively correlated with patient's safety culture recognition, patient safety knowledge, and incident reporting attitude(p<.001), and patient safety nursing activities are found to be highly performed compared to the low-level group with high recognition of patient safety culture(p<.001). And also, patient safety nursing activities were found to be highly performed compared to the low-level group with the high-level group of patient safety knowledge and incident reporting attitude(p<.001).

    연구목적: 요양병원 간호사의 환자안전문화인식, 환자안전지식, 사건보고태도에 따른 환자안전간호활동 수행의 차이를 분석함으로써, 요양병원 간호사들을 대상으로 환자안전간호활동 교육프로그램 개발을 위한 기초자료를 제시하고자 수행된 서술적 조사연구이다.
    연구방법: 본 연구 대상자는 G도 C시에 소재한 요양병원에 근무하는 간호사 188명이었다. 대상자의 선정기준은 현재 재직 중인 요양병원에서 최소 3개월 이상 근무한 경력이 있는 간호사로 본 연구의 참여에 자발적으로 서면 동의한 간호사이었다. 자료수집 기간은 2018년 7월 1일부터 7월 30일까지였으며, 연구자가 직접 설문에 대하여 설명하고 서면동의서를 받은 후 대상자가 직접 작성하게 하는 자가보고식 설문지법을 이용하여 자료를 수집하였다. 198명의 간호사를 대상으로 설문조사를 실시하였으나 불성실한 답변과 답을 하지 않은 문항이 있는 설문을 제외한 후 총 188명의 자료를 SPSS 23.0 프로그램을 이용하여 최종 분석하였으며 그 결과는 다음과 같다.

    2. 대상자의 환자안전문화에 대한 인식 점수는 5점 만점에 3.84점, 환자안전에 대한 지식은 5점 만점에 3.82점, 사건보고에 대한 태도 점수는 5점 만점에 3.70점이었다. 환자안전문화인식 중 조직차원에 대한 하위영역별 인식은 리더십 4.03점, 환자안전정책및절차 3.78점, 환자안전개선시스템 3.64점으로 조직의 리더십에 대한 인식점수가 가장 높았으며 조직의 환자안전개선시스템에 대한 인식점수가 가장 낮았다. 부서차원에 대한 하위영역별 인식은 팀워크 3.92점, 비처벌적환경 3.69점이었고, 개인차원에 대한 하위영역별 인식은 환자안전지식과태도 4.03점, 환자안전우선순위 3.41점으로 환자안전 우선순위에 대한 인식이 낮았다. 사건보고태도에 대한 하위영역은 평가활용에대한우려 3.41점, 개선효과에대한신념 3.91점, 보고의도 4.00점으로 보고의도에 대한 태도가 높게 나타났다. 대상자의 환자안전간호활동 수행은 5점 만점에 4.65점으로 대체로 높게 나타났다.

    3. 환자안전문화인식은 대상자의 학력에 따라 유의한 차이를 보였고, 환자안전지식은 대상자의 나이, 실무경력, 현재직급, 근무형태, 병원의 인증평가 경험 유무에 따라 유의한 차이를 보였다. 사건보고태도는 대상자의 종교, 총 실무경력, 요양병원 근무경력, 직급, 근무형태에 따라 유의한 차이가 있었고, 환자안전간호활동은 대상자의 나이, 결혼상태, 학력, 실무경력, 직급에 따라 유의한 차이가 있는 것으로 나타났다.
