서론: 아데노바이러스(HAdV)는 전 세계적으로 소아 호흡기 감염의 주요 원인 병원체이며, 전장유전체분석(whole-genome sequencing, WGS)은 바이러스의 유전적 다양성을 규명하는데 필수적인 도구로 자리 잡고 있다. 아데노바이러스 3형(HAdV-3)은 국내 소아에서 수십 년에 걸쳐 반복적인 유행을 일으켜 온 주요 원인 바이러스이나, 장기간에 걸친 유전체수준의 변이 범위와 그 특성에 대해서는 아직 충분히 규명되지 않았다. 이에 본 연구에서는 1991년부터 2024년까지 국내 소아에서 분리된 HAdV-3 분리주를 대상으로 전장유전체 분석을 수행하여, HAdV-3의 지속적인 유행과 관련된 유전체적 특성을 규명하고자 하였다.
방법: 1991년부터 2024년까지 서울대학교 어린이병원에서 호흡기 감염으로 진단된 소아 환자의 호흡기 검체로부터 배양된 HAdV를 대상으로 유형 분포를 분석하였다. 2007년 이전에 분리된 검체는 혈청 중화시험 또는 다중중합효소연쇄반응(multiplex PCR)을 통해 유형을 결정하였으며, 2008년 이후 검체는 hexon 유전자의 과변이 영역(HVR1–6) 염기서열 분석을 통해 유형을 분류하였다. 주요 유행 시기를 대표하는 35주의 HAdV-3 분리주를 선정하여 전장유전체 분석을 수행하였다. 국내 분리주 간 유전체 비교 분석을 통해 유전적 다양성을 평가하였으며, 전 세계적 비교를 위해 1953년부터 2024년까지 6개 국가에서 보고된 122주의 HAdV-3 완전 유전체 서열을 포함하여 neighbor-joining 기반 계통유전학적 분석을 수행하였다.
결과: 총 1,235주의 HAdV가 분리되었으며, A–F의 6개 종(species)과 18개 유형(type)으로 분류되었다. 이 중 B종(60.9%)과 C종(33.2%)이 우세하였고, 가장 빈번한 유형은 HAdV-3(42.5%), HAdV-7(14.7%), HAdV-2(14.3%), HAdV-1(11.2%) 순이었다. HAdV-3는 약 2–3년 간격의 반복적인 유행을 보이며 연구 기간 전반에 걸쳐 가장 우세한 유형으로 유지된 반면, HAdV-7은 1990년대 후반 폭발적인 유행 이후 장기간 검출되지 않았다. 코로나19 대유행 기간(2020–2021년) 동안 HAdV 검출은 거의 소실되었으나, 이후 2023년에 HAdV-3에 의해 급격히 재증가하였다.
전장유전체 분석 결과, 국내 HAdV-3 분리주들은 서로 간에 99.5% 이상의 높은 염기서열 일치도를 보였으며, 원형주(prototype)인 GB 균주와도 98% 이상의 높은 일치도를 나타내어 전반적으로 매우 안정적인 유전체 구조를 유지하고 있었다. 전 세계 HAdV-3 유전체와의 비교에서도 염기서열 일치도는 전반적으로 높았으며(>98%), 미확정 염기가 많은 최근 러시아 분리주 1주를 제외하면 이러한 경향은 일관되게 유지되었다. 국내 분리주 간Sliding-window 분석과 SimPlot 분석 결과에서도 대규모 재조합의 증거 없이 고도로 보존된 유전체 골격이 확인되었다. 유전자 기반 SNP 밀도는 초기 및 조절 유전자 및 부 구조 단백질 유전자에 상대적으로 집중되어 있었다. 주요 구조 단백질 유전자(penton base, hexon, fiber)에서는 제한적인 아미노산 치환만 확인되었으며, 35주 분리주 중 penton base에서 11개, hexon에서 23개, fiber에서 9개의 비동의적 아미노산 치환이 확인되었다. 계통학적 분석에서는 국내 HAdV-3 분리주들은 짧은 분지 길이를 갖는 조밀한 군집을 형성하였으며 시간에 따른 미세한 아형 분화만을 보였다.
결론: 본 연구는 전장유전체 기반 분석을 통해, HAdV-3가 국내 소아에서 30년 이상 반복적인 대규모 유행을 주도해 왔음에도 불구하고, 고도로 보존된 유전체 골격을 유지하고 있음을 확인하였다. 지속적인 우세에도 불구하고 HAdV-3에서는 유전체 수준의 뚜렷한 다양화나 재조합은 관찰되지 않았다. 비록 본 연구 결과는 대표 분리주를 기반으로 한 분석이라는 점에서 해석에 주의가 필요하나, 이러한 유전체적 안정성은 새로운 바이러스 변이의 출현이 국내 HAdV-3 반복 유행의 주요 원인이 아닐 가능성을 시사한다. 향후 연구에서는 HAdV-3의 장기적인 유행 특성을 규명하기 위해 숙주 집단 면역과 전파 역학을 중심으로 한 접근이 필요하다.

Background: Human adenoviruses (HAdVs) is one of the major pediatric respiratory pathogens worldwide, and whole-genome sequencing (WGS) has become essential for characterizing genome-wide viral diversity. Human adenovirus type 3 (HAdV-3) has remained a predominant cause of recurrent epidemics over several decades in Korea, yet the nature of its long-term genomic variation remains poorly understood. To address this gap, we conducted a comprehensive WGS-based analysis of HAdV-3 isolates collected from Korean children over a 34-year period (1991–2024), to define the genome-wide variation underlying its sustained circulation.
Methods: HAdVs cultured from respiratory specimens collected from children with respiratory infections at Seoul National University Children’s Hospital between 1991 and 2024 were analyzed for type distribution. For isolates collected before 2007, types were assigned based on serum neutralization assays or multiplex PCR. Isolates collected since 2008 were typed by sequencing the hexon hypervariable regions (HVR1–6). Thirty-five representative HAdV-3 isolates from major epidemic periods spanning 1991 to 2024 were selected for WGS. Genome-wide comparative analyses were performed to characterize genetic diversity among Korean HAdV-3 isolates. For global comparison, neighbor-joining–based phylogenetic analyses were conducted in conjunction with 122 publicly available HAdV-3 complete genome sequences retrieved from the NCBI GenBank database, representing isolates from six countries spanning 1953–2024.
Results: A total of 1,235 HAdV isolates comprising six species (A–F) and 18 types were identified. Species B (60.9%) and species C (33.2%) predominated. The most frequent types were HAdV-3 (42.5%), HAdV-7 (14.7%), HAdV-2 (14.3%), and HAdV-1 (11.2%). HAdV-3 caused recurrent outbreaks at 2 to 3-year intervals and remained the dominant epidemic type throughout the study period, whereas HAdV-7 was characterized by explosive outbreaks in the late 1990s, followed by prolonged absence from detection. During the COVID-19 pandemic (2020–2021), HAdV circulation nearly ceased, followed by a marked resurgence in 2023 driven almost exclusively by HAdV-3.
Whole-genome analyses demonstrated remarkable genetic stability of HAdV-3, with >99.5% nucleotide identity among Korean isolates and >98% identity relative to the prototype GB strain. Whole-genome nucleotide identity between Korean isolates and globally reported HAdV-3 genomes was consistently high (>98%), with the exception of a single recently deposited Russian strain that contained extensive ambiguous bases. Among Korean isolates, sliding-window and SimPlot analyses revealed a highly conserved genomic backbone without evidence of large-scale recombination. Gene-based SNP density was preferentially concentrated in early regulatory and minor structural genes. Major structural protein genes (penton base, hexon, and fiber) showed a limited number of amino acid substitutions, with 11, 23, and 9 nonsynonymous substitutions identified in the penton base, hexon, and fiber genes, respectively, among 35 HAdV-3 isolates. Phylogenetic analyses showed that Korean HAdV-3 isolates formed a compact cluster with short branch lengths and only minor chronological sub-clustering.
Conclusions: This long-term genome-based study demonstrates that HAdV-3 has persistently driven recurrent epidemics among Korean children for more than three decades while maintaining a highly conserved genomic backbone. Despite its sustained predominance, HAdV-3 exhibited limited genome-wide variation rather than substantial diversification or recombination. Although these findings are based on a selected set of representative strains and should be interpreted with caution, the emergence of novel viral variants is unlikely to be the primary driver of recurrent HAdV-3 outbreaks in Korea. Future research should therefore focus on host population immunity and transmission dynamics that shape the long-term epidemic behavior of HAdV-3.
