This study aimed to explore the key medium characteristics related to the counseling relationship experienced by clients in online video counseling and to analyze the issues associated with these characteristics. Using Timulak’s (2009) qualitative meta-analysis method, this study analyzed eight research articles on client experiences in online video counseling published in domestic KCI-indexed journals from 2021 to the first half of 2024. The analysis examined the medium-specific characteristics reflected in clients’ relational experiences in online video counseling and the resulting issues. The results revealed that clients’ relational experiences in online video counseling were associated with three main medium characteristics, leading to a total of three corresponding issues. First, “the issue of safety and anxiety arising from physical separation”; second, “the issue of therapist presence and its resulting impact”; and third, “the issue of interaction characteristics and the degree of engagement.” The findings suggest that online video counseling is not merely a substitute for face-to-face counseling but represents an independent form of counseling with its own unique relational characteristics and issues. Based on these results, approaches including therapeutic utilization of the dual nature of physical separation, reconceptualization of therapist presence, and promoting client agency were discussed.
