침습적 외상 기억과 외상 증상 및 심리적 부적응에 대한 경험 회피와 해리의 관계

국문 초록 (Abstract)

본 연구에서는 외상 증상의 구조적 이해에 수용 중심적 접근에서 핵심 변인으로 제시하고 있는 경험회피의 작용을 확인하고자 외상적 사건경험이 있는 서울, 경기 지역의 일반 성인 528명을 대상으로 하여 침습적 외상 기억과 심리적 부적응, 침습적 외상 기억과 외상 증상 관계에 경험 회피와 해리의 매개효과를 검증하였다. 연구 참여자들은 다양한 영역에서의 외상적 사건 경험을 측정하는 외상적 사건 경험 질문지, 외상 증상 평가를 위한 외상 후 진단 척도, 침습적 외상 기억을 측정하기 위한 IES-R-K, 원치 않는 사적 경험에 접촉하지 않으려는 정도를 측정하기 위한 AAQ-II, 해리 증상 존재여부와 그 정도를 측정하기 위한 해리 경험 척도, 심리적 부적응 수준을 측정하기 위한 간이증상도구(BSI-18)으로 구성된 총 94문항의 자기보고식 설문을 실시하였다. 구조방정식 모형을 분석한 결과, 해리와 외상 증상을 제외한 모든 변인 간 관련성이 유의한 것으로 나타났다. 구조 모형에 대한 개별 매개 경로의 유의성을 확인해보면 경험 회피는 침습적 외상 기억과 외상 증상, 침습적 외상 기억과 심리적 부적응 수준에 영향을 미치는 것으로 나타났다. 또한 침습적 외상 증상과 심리적 부적응의 관계에 경험 회피와 해리가 영향을 미치는 것으로 나타났다. 반면, 침습적 외상 기억과 외상 증상의 관계에서 경험 회피와 해리는 매개하지 않는 것으로 나타났다. 이에 연구 결과를 바탕으로 경험 회피가 외상 경험자들의 심리적 증상에 미치는 영향력을 파악하고 외상 관련 상담 실제에 대한 시사점과 후속 연구에 대한 제언을 논하였다

다국어 초록 (Multilingual Abstract)

The purpose of this study was to examine the mediating effects of experiential avoidance and dissociation on the relationship between intrusive memory and traumatic symptoms/ psychological distress. The research model was set up based on the previous research that experiential avoidance works as a vulnerability of traumatic symptoms and psychological distress, which is the basic theory of acceptance based approach. To examine the relationship among variables and role of experiential avoidance which is the core variable of the Acceptance-Based Treatment, survey data from 528 adults exposed to traumatic events was analyzed using structural equation modeling (SEM). The results of data analysis were summarized as follow: First, the results indicated that each variable was positively related, but dissociation was not significantly associated with traumatic symptoms. Second, in this research model, experiential avoidance has an mediated effect on the relationship between intrusive memory and traumatic symptoms, and intrusive memory and psychological distress. Third, experiential avoidance and dissociation significantly mediated the effects of intrusive memory on psychological distress simultaneously. On the other hand, the relationship between intrusive memory and traumatic symptoms was not significantly mediated by experiential avoidance and dissociation at once.
