정신장애와 정신분열 대상의 ICF core set 개발을 위한 탐색적 연구 : “활동과 참여” 및 “환경요인”을 중심으로 = An Exploratory Study on ICF Core Set Development for People with Mental Disabilities and Schizophrenia. -Focused on "Activity and Participation" and "Environmental Factors"-

Participation in social activities by people with mental disabilities have been restricted by functional impairment due to their underlying diseases. However, based on the International Classification of Functioning, Disability, and Health(ICF),endorsed by World Health Organization(WHO), a patient’s health can be positively affected by environmental factors, irrespective of limitations on their performance capacity. Therefore, the definition of disability from the ICF classification has been recommended to be used in many countries. This study therefore examined a index of two-level classification and defined by a core set of indices exploring parameters for activity, participation and environmental factors to people with mental disabilities and schizophrenia. We used the Delphi technique in three rounds performed on two different sessions. During the study, we collected data on the cohort of mental disabiled participants in the first session; and subsequently collected data restricted to a sub-cohort of schizophrenic patients in the second session. After analysis of the data, we were able to associate the activity and participation patterns of people with mental disabilities and schizophrenia. In addition, the adequacy of the ICF 2-level classification index was analyzed to provide correlations in developing a core set of associated parameters. Based on the results of the analysis regarding activity and participation, people with mental disabilities seem to have difficulties in interpersonal interactions and relationships, domestic and community life. In environmental factors, we conclude that the people suffering from schizophrenia were much more seriously affected by external attitudes, showing that mental disabilities needs more positive facilitators such as supportive relationships, services, systems and policies. Analyzing the adequacy of ICF 2-level classification index, we obtained 114 parameters(72 for activity and participation; 42 for environmental factors) as potential associated indices for explaining mental disability and schizophrenia, which could be used as the basis for the development of a core set of correlative parameters. Even if more factors are to be considered in future studies, this study contributes to the fundamental understanding of social rehabilitation, vocational rehabilitation/training and accompanying social policies, without the modifications of ICF by WHO.

국문 초록 (Abstract)

본 연구는 정신장애와 정신분열 대상으로 세계보건기구(WHO)가 승인한 ICF(International Classification of Functioning, Disability, and Health) 분류 체계 중 활동과 참여 및 환경요인에 해당하는 core set을 도출하고자 2단계 분류의 지표를 탐색해 본 것이다. 연구방법은 각 3회기로 이루어진 2차의 델파이 조사로 이루어졌다. 1차 조사는 정신장애 전반에 관한 자료를 수집하였고, 2차 조사는 정신분열 대상으로 유형을 제한하여 자료를 수집하였다. 수집된 자료를 통해 정신장애와 정신분열 대상의 활동과 참여, 환경요인의 영향력을 알 수 있었으며, ICF 2단계 분류 중에서 적절한 지표를 파악하고, core set 도출을 위한 코드를 제시하였다. 연구결과 ICF 2단계분류 중에서 정신장애 및 정신분열을 설명하는 적절한 지표로 활동과 참여 72개, 환경요인 42개로, 총 114개 코드가 도출되었다. 본 연구는 core set 개발을 위한 탐색적 연구라는 제한점은 있으나, WHO의 ICF 틀과 그 문항들을 변형하지 않고 적용한 연구로, 국제기준에 맞는 사회재활 및 직업재활과 훈련, 그리고 그에 따른 사회 정책과 관련 프로그램에 대한 제언을 위한 기초자료가 될 수 있을 것이다.
