본 연구는 장기기증에 동의한 가족을 대상으로 뇌사와 장기기증과 관련한 지식, 태도, 기타 요인들을 알아보고, 이를 바탕으로 향후 뇌사자 장기기증 활성화 방안을 마련하기 위한 기초자료를 제공하는 것을 목적으로 실시되었다.
연구대상은 2000년부터 2003년까지 국립장기이식센터에 등록된 장기기증자의 가족(총 174명)으로 하였고, 구조화된 설문지를 개발하여 이용하였으며, 우편설문 방법을 사용하였다. 연구 대상자에게 사전에 전화를 통해 연구 목적과 취지를 설명하고, 동의를 얻은 후 연구에 동의한 대상자(총 168명)에 한해 우편 설문지를 발송하였다. 총 168부 중 회수된 78부(회수율 44.8%)를 최종 연구집단(Study population)으로 선정하여 분석하였다. 또한 추가적으로 같은 기간에 계통표본 추출 방법으로 추출된 서울, 경기 지역 주민 255명을 대상으로 면접 설문을 실시하였다.
본 연구에서는 실제로 장기를 기증한 뇌사자의 가족들을 대상으로 그들이 장기 기증에 동의하는데 영향을 미치는 변수들을 1. 인구학적 특성 2. 가족의 뇌사에 대한 이해도 및 장기기증에 대한 태도 3. 병원 및 의료 서비스 관련 변수 4. 장기기증 요청 과정 5. 사회정서적 및 의사소통 과정 6. 가족내 논의 7. 가족내 의사 결정 8. 뇌사자 특성의 8가지 요인으로 구분하여 조사하였다.
연구 결과 장기 기증에 동의한 가족의 뇌사에 대한 이해도는 높은 편이었으며, 장기기증에 대해서도 긍정적인 태도를 가지고 있음을 알 수 있었다. 뇌사 판정기준을 알고 있는 경우는 전체의 65.4%였으며, 장기 기증시 가족 2인 이상의 동의가 필요함을 알고 있는 경우도 전체의 61.5%로 나타났다. 또한 뇌사를 죽음으로 인정하는 것을 찬성하는 가족이 전체의 87.2%로 나타나 뇌사 인지도가 상당히 높음을 알 수 있었다. 반면에 장기 기증의 배분과정이나 장기 기증 절차에 대한 인지도는 낮은 편으로 조사되어 뇌사자 가족들에게 장기기증 절차나 배분 과정 등에 관한 정보를 제공하는 것이 부족한 실정임을 알 수 있었다.
장기 기증에 관한 태도에서도 본인이 뇌사 상태에 처했을 경우 장기기증에 찬성하는지를 묻는 문항에서는 응답자의 76.9%가 찬성한다고 답해 일반인을 대상으로 한 기존 연구들에서의 결과 보다 높은 비율을 나타냈다. 가족이 뇌사상태에 처했을 경우에는 74.4%가 찬성한다고 답했으며, 잘모르겠다 라고 답한 경우가 20.5%로 자신이 뇌사상태에 처한 경우보다 가족이 뇌사 상태에 처하여 장기 기증을 할 경우 좀 더 신중한 경향을 보였다.
장기 기증 요청 과정의 경우 대부분 가족 스스로 결정한 경우가 많았으며, 그 밖에 의료인이나 장기이식코디네이터 순으로 장기 기증을 권유했다고 응답했다. 요청 없이 가족 스스로 장기 기증을 결정했다는 것은 각 병원에서 장기기증자 발굴에 소극적 태도를 가지고 있음을 의미하며, 뇌사 발생 후 장기 기증을 요청하는 일련의 과정이 원활하게 이루어지고 있지 못함을 뜻한다.
장기기증자 생전에 가족들과 장기기증에 관해 서로 논의한 적이 있는가를 묻는 문항에서는 전체 응답자의 75.6%가 논의한 적이 없었다고 답했으며 가족들이 생전에 장기기증자의 기증 의사나 의견을 충분히 알고 있었다고 생각하는가를 묻는 문항에서는 알지 못한다는 의견이 전체의 61.5%를 차지해 아직까지 장기 기증에 대한 인식이 많이 부족하고 활발한 논의가 이루어지지 않고 있음을 알 수 있었다. 또한 장기기증 활성화를 위한 구체적인 방안 마련이 시급함을 알 수 있었다.
장기 기증에 관한 동의를 내리기까지 소요된 의사결정 시간은 3일 이내의 경우가 64.1%로 가장 많았고, 가족들의 논의 횟수는 3회 이하가 64.1%로 가장 많았다. 장기 기증에 동의하는 최종 결정을 내린 사람은 뇌사자의 부모가 31명(39.7%)로 가장 많았으며, 배우자가 19명(24.4%), 뇌사자의 형제, 자매가 16명(20.5%) 뇌사자의 자녀가 8명(10.3%) 순으로 나타났다.
장기 기증에 관해 협의 할 당시 가족 중 강력한 의사결정자가 있었는가 하는 문항에서 강력하게 장기 기증을 주장한 사람이 있었다고 응답한 경우가 75.6%로 반대한 사람의 경우 35.9% 보다 높은 비율을 나타내었다. 이 같은 사실은 장기 기증에 있어 가족 중 의사결정자의 역할과 가족 내 의사결정 과정이 상당히 중요함을 의미한다. 가장 강력한 의사결정자로 찬성한 사람은 부모 24명(30.8%), 형제 17명(21.8%), 배우자 11명(14.1%) 순으로 나타났으며, 반대했던 사람은 기타 친인척 15명(19.2%), 부모 6명( 7.7%), 형제 4명(5.1%) 순으로 나타났다.
본 연구 결과를 종합해 볼 때, 장기기증 부족 문제를 해결하고 장기기증을 활성화하기 위해서는 무엇보다 뇌사자 가족들의 장기기증에 대한 긍정적인 인식의 형성이 우선되어야 함을 알 수 있었다. 그리고 뇌사자 가족에게 장기 기증 절차나 과정에 대한 충분한 지식이 제공되지 못하고 있으며, 뇌사 발생 후 장기 기증을 요청하는 과정과 의사소통 과정이 제대로 이루어지고 있지 못함을 파악할 수 있었다. 또한 각 병원에서의 뇌사자 발굴을 위한 노력이 부족하며, 이 같은 의료인의 소극적인 태도를 개선하기 위한 인센티브 마련이 시급함을 알 수 있었다.
자료의 제한으로 인해 뇌사자 중 장기기증을 하지 않은 집단을 연구에 포함시키지 못한 것은 본 연구의 가장 큰 한계점으로, 앞으로 이 부분에 대한 연구가 추가적으로 시행되어 어떠한 요인이 장기 기증 동의에 결정적인 역할을 하는지 연구되어야 할 것이다. 이와 같은 연구를 통해 향후 긍정적인 영향을 주는 요인들을 촉진시키고, 부정적인 영향을 감소시키는 방향의 중재를 마련할 수 있을 것이며, 나아가 장기기증의 활성화 방안에 필요한 홍보, 교육의 기초 자료로 활용할 수 있을 것이다.

The purpose of this research is to offer basic source materials for a plan to activate organ donation from the patients with brain death, by examining knowledge of family members who consented to make an organ donation of their family member about brain death, organ donation, their attitude and other facts.
A structured and developed questionnaire was sent via mail to 168 out of total 174 family members of donors listed in the Korean Network for Organ Sharing from 2000 to 2003. Before we sent out questionnaire, we called each individual and explained the purpose of the research and only those who agreed to participate received questionnaire by mail. Of the 168 questionnaires sent out, a total of 78 participants took part in the survey yielding a response rate of 44.8 % and they were selected as a final study population and analyzed.
Followings are 8 variables in this study which affect family members' decision to consent to the organ donation. 1. demographic characteristics, 2. family member''s understanding on brain death and their attitude to organ donation, 3. variables related to hospital and medical service, 4. process of requesting organ donation, 5. social feeling and process of communication, 6. family discussion, 7. family decision, 8. traits of a patient with brain death.
Results provide that family members' understanding about brain death was pretty high and their attitudes to organ donation were positive. 65.4% of participants knew criterion of brain death and 61.5% of them knew that at least two of the family members have to sign on the consent in order for the organ donation. Also, 87.2% of them supports the idea which consider brain death as a death implying high recognition about brain death. On the other hand, understanding of distribution process of organ donation was low which signifies lack of supplying information to the family members.
76.9% of participants claimed that they would like to donate their organ when their brain is dead and the rate is higher than that of public. In addition, 74.4% of participants claimed that they will agree to consent when their family members are in brain death state and 20.5% responded with "do not know." Which shows the careful tendency when they make decisions on their family members.
For the process of requesting organ donation, many of the cases, the decisions were made by the family members. Physicians and organ coordinator in order asks for organ donation. It means that hospitals do passive roles in finding organ donors and the process of requesting organ donation made after the event of brain death does not be made well.
To the question asking if there was a discussion between family members and the donor about organ donation when the donor was alive, 75.6% of participants responded with "no" and 61.5% of participants claimed to "do not fully aware of the opinion of donor." These results indicate that there aren't active discussions about organ donation and there is a lack of recognition about organ donation. Furthermore, there must be an immediate plan for the active organ donation.
From the result, the majority of respondents (64.1%) answered that the decision making time took less than three days and also 64.1% of respondents claimed to have had family discussion less than 3 times. Regarding about the final decision maker about organ donation, 31(39.7%) respondents indicated "parents of donor," 19(24.4%) respondents indicated "spouse of donor," 16(20.5%) respondents indicated "sibling of donor," and 8(10.3%) respondents indicated "children of donor."
A response to the question if there was a strong decision maker among the family members, 75.6% of respondents claimed to have a strong decision maker who insists to donate. Conversely, 35.9% of respondents claimed to have one who is strongly negative to organ donation. According to the facts, organ donation is significantly associated with the role of decision makers among the family members and the process of decision making. For the positive strong decision makers, 24(30.8%) respondents indicated "parents of donor," 17(21.8%) respondents indicated "sibling of donor," and 11(14.1%) respondents indicated "spouse of donor." Conversely, for the negative ones, 15(19.2%) indicated "relatives," 6(7.7%) indicated "parents" and 4(5.1%) indicated "sibling".
Overall with the results of this study, to solve the lack of organ donation and to activate organ donation, positive way of thinking of family member to the organ donation is the priority. In addition, results showed that there is a shortage of information to the family members about process and procedure of organ donation and that communication processes and requests for the organ donation after the event of brain death do not work. Furthermore, no efforts are made to find the patients with brain death. To ameliorate the passive attitude of physicians, setting an incentive system is readily required.
Sampling of this study is limited to family members of donor and not including the group of family members who have not donors. Further research is needed to figure out which variable affects the most on the organ donation decision making. This research and further researches will provide indicator which helps to stimulate the important positive factors and to diminish the important negative factors in decision making. Furthermore, those studies can be used as basic knowledge in marketing and education to activate the organ donation plan.
