Bladder cancer is one of the most common malignancies of the urinary tract and is characterized by a high recurrence rate, requiring continuous monitoring and repeated diagnostic testing. Although cystoscopy is considered the gold standard for the diagnosis and surveillance of bladder cancer, the procedure is invasive and associated with patient discomfort and high medical costs. Therefore, there has been increasing interest in the development of non-invasive urinary biomarker-based diagnostic methods.
In this study, a combo diagnostic kit capable of simultaneously detecting the bladder cancer biomarkers NMP22 and UBC (CK8) was developed, and its analytical performance was evaluated. For the isolation and purification of native antigens, bladder cancer cells were cultured and cell extracts were prepared. Monoclonal antibodies showing high reactivity against NMP22 and UBC (CK8) were selected through ELISA-based analysis. The selected antibodies were conjugated to cellulose resin to prepare antibody-resin complexes, and affinity columns specific for each biomarker were constructed. Using these affinity columns, highly purified native antigens were isolated and purified. The purified native antigens were subsequently applied to the development of the combo diagnostic kit.
The developed kit was designed in a lateral flow immunoassay format for the simultaneous detection of NMP22 and UBC (CK8) in urine samples. Analytical performance evaluation was conducted according to guidelines for in vitro diagnostic medical devices, including limit of detection, reproducibility, stability, cross-reactivity, and interference tests. In addition, spiked urine samples containing purified native antigens were used to evaluate the practical applicability of the developed kit.
The results demonstrated that the developed combo diagnostic kit successfully detected both NMP22 and UBC (CK8) simultaneously without significant cross-reactivity or interference. Stable and reproducible detection performance was confirmed, and positive reactions were observed even at low concentrations in spiked urine samples.
These findings suggest that the developed combo diagnostic kit may serve as a useful non-invasive diagnostic tool for the simultaneous detection of bladder cancer biomarkers. Furthermore, the use of highly purified native antigens isolated through antibody-based affinity purification may contribute to improving the analytical performance and practical applicability of bladder cancer diagnostic assays.
