중환자 가족과의 효과적인 의사소통을 위한 간호정보 템플릿 개발

Background: When a patient is admitted to Intensive Care Unit (ICU), various needs occur from his or her family. In particular, as the need for information increases, appropriate information about ICU patients should be provided in an effective and friendly way to meet their information needs. Recently, mobile devices such as smart phones, tablet PCs are used in the healthcare field and research on how to provide information about ICU patients, using mobile devices has been increased.
Objective: This study aims to explore the needs of nursing information from family members of ICU patients and develop a nursing information template for effective communication with family members of ICU patients, based on their needs.
Methods: This is a methodological research to develop a nursing information template.. The needs for nursing information were explored through literature reviews and interviews with ICU nurses and family members of ICU patients. Based on the information needs, the items of nursing information were organized. The final items of nursing information were selected through content validity index by experts. After a tablet PC-based nursing information template was drafted, it’s content, design and strategy was assessed by conducting a survey with nurses. The nursing information template was finalized, on the basis of the assessed items.
Results: Among 13 nursing information items selected through literature reviews and interviews, 11 items were finalized based on CVI of 0.80 or above. The template was organized to be available on a table PC and its draft for nursing information was developed and easy terms for helping understanding of families with ICU patients. The initially developed template was evaluated by 20 ICU nurses and a total score was 4.13 of 5 points. In particular, scores of contents (4.30± 0.56) and strategies (4.18± 0.79) were high and a score of template design was 3.90. Based on nurse’s evaluation, the font size was increased and duplicate information was merged.
Conclusion: The nursing information template, which was developed in this study, would be a useful tool to satisfy the information needs of family members of ICU patients. Moreover, it is expected to promote participation of families in patient care and improve in an effective way of communication between ICU nurses and families.

국문 초록 (Abstract)

본 연구는 중환자 가족의 간호정보 요구를 파악하고 이를 기초로 간호정보 템플릿을 개발하기 위한 방법론적 연구이다. 이 템플릿의 사용으로 중환자 가족의 눈높이에 맞추어 간호정보를 제공함으로서 중환자 가족과의 효과적인 의사소통을 가능하게 하며, 중환자 간호에 가족이 참여하도록 유도하기 위한 기초연구로 시도되었다.
본 연구는 중환자 가족의 정보요구와 관련된 문헌고찰을 토대로 치료, 환자상태, 일일계획, 안위수준, 가족참여 5개의 영역으로 분류 하였고, 이를 바탕으로 중환자 가족의 간호정보 요구 조사를 위한 반 구조화된 질문을 구성하였다. 2016년 4월 15일부터 5월 10일 까지 서울 소재의 일개 대학 병원의 중환자실 간호사 10인과 중환자 가족 10인을 대상으로 개별 면담을 시행하였다. 그 결과 요구도 50%이상인 13개의 예비 간호정보 제공 항목을 선정하였으며, 이에 대해 8명의 전문가를 대상으로 내용 타당도를 파악하여 CVI 0.80 이상인 간호정보 제공 항목을 선정하였고, 전문가의 의견에 따라 항목의 내용을 추가하거나 수정하여 11개의 간호정보 제공 항목을 선정하였다.
선정된 항목에 대해 간호정보 제공을 위한 태블릿 PC 기반의 템플릿 초안을 개발하였다. 초안에 대해 중환자실 간호사 20인에게 간호정보 템플릿의 내용, 설계, 전략에 대한 설문을 시행하였다. 설문결과 간호정보 제공을 위한 템플릿 초안의 총 평가는 5점 만점에 평균 4.13점이었고, 하부 항목인 내용은 평균 4.30점, 설계는 평균 3.90점, 전략은 평균 4.18점으로 평가되었다. 중복되는 정보는 하나로 통합하면 좋겠다는 의견과 글자의 크기가 작다는 등의 의견을 반영하여 수정 및 보완 하는 단계를 거쳐 최종 간호정보 템플릿을 개발하였다.
본 연구에서 개발된 간호정보 템플릿은 실제 임상에서 적용하여 환자의 상태에 따른 그날의 간호정보를 제공하여 중환자 가족의 정보요구를 충족시켜 줄 수 있는 효과적인 방법이 될 것이라 생각된다. 더불어 중환자실 간호사와 중환자 가족의 의사소통 어려움을 개선하며, 환자 치료에 가족참여를 유도하여 궁극적으로는 중환자 가족의 불안, 우울과 같은 심리적 증상 경감과 중환자 치료에 긍정적인 영향을 줄 것으로 사료된다.
