혈액투석환자의 자가관리 이행 영향요인 = Factors Influencing Self-Management Adherence in Hemodialysis Patients

This is a descriptive correlation study to identify depression, uncertainty, self-care agency, and health literacy as factors that affect self-management adherence in hemodialysis patients. From 10 January to 15 February 2021, a structured questionnaire was used to collect data from 123 patients in two artificial kidney rooms in S city and G city of the J province. The data were analyzed using the SPSS/WIN 25.0 program for descriptive statistics by applying independent t-test, one-way analysis of variance, Pearson's correlation coefficient, and multiple regression. Self-management adherence was negatively correlated with depression (r=-.21, p=.019) and uncertainty (r=-.30, p=0.001) and positively correlated with self-care agency (r=.58, p<.001). Factors that significantly affected self-management adherence in hemodialysis patients were marital status (β=.18, p=.040) and cognitive aspect of self-care agency (β=.38, p=.016), with an explanatory power of about 48.8% (F=8.75, p<.001). In order to increase the self-management capacity of hemodialysis patients, our results indicate the need to develop a target education program that involves the spouses in the nursing intervention program and enhances the cognitive aspect of self-nursing competency.

국문 초록 (Abstract)

본 연구는 혈액투석환자의 자가관리 이행에 미치는 영향요인 중 우울, 불확실성, 자가간호역량 및 건강정보이해능력을 파악하고 자가관리 이행에 영향을 미치는 요인을 규명하기 위한 서술적 상관관계 연구이다. 연구대상은 J도의 S시와 G시 인공신장실에서 혈액투석 치료 중인 환자 123명으로 자료수집은 2021년 1월 10일부터 2월 15일까지 실시되었다. 자료는 SPSS/WIN 25.0 program으로, independent t-test, one-way ANOVA, Pearson’s correlation, multiple regression으로 분석하였다. 혈액투석환자의 자가관리 이행은 우울(r=-.21, p=.019), 불확실성(r=-.30, p=.001)과 부적 상관관계, 자가간호역량(r=.58, p<.001)과는 정적 상관관계가 있었으며, 가장 큰 영향요인은 자가간호역량 중 인지적 측면(β=.38, p=.016)이었으며, 결혼상태 중 배우자 있음(β=.18, p=.040)이 두 번째로 유의한 영향요인으로 나타났고(F=8.75, p<.001), 48.8%의 설명력을 보였다. 본 연구결과를 바탕으로 혈액투석환자의 자가관리 이행을 높이기 위해서는 간호중재 프로그램에 배우자를 참여시키며, 인지적 측면의 자가간호역량을 증진할 수 있는 대상자 교육 프로그램 개발이 필요할 것으로 보인다.
