문제해결식 전화상담이 치매노인 가족간호제공자의 상황평가와 대처기전에 미치는 효과 = Effects of the Problem-Solving Telephone Counseling on Caregiving Appraisal and Coping in Family Caregivers of the Older Adults with Dementia

The purpose of the study is to identify the effects of problem-solving telephone counseling on appraisal and coping in family caregivers of the older adults with dementia. Using a computer system, a random assignment of subjects was conducted from January to October, 2002 in Pusan. A total of 53 family caregivers were included in the study and 21 caregivers received telephone counseling once a week for 12 weeks and 32 caregivers received general information about caring older adults with dementia. Measures were included Appraisal of Caregiving Scale and Coping Strategies Inventory. T-test were used to identify significant differences between groups. The findings are as follows: 1)There were no significant differences between the two groups on appraisal and coping after telephone counseling. 2)when the effects of telephone counseling were compared separately for spouse and child caregivers, spouse caregivers under the telephone counseling utilized problem focused coping more often than the comparison group at the post-test. However, telephone counseling did not make any significant differences on appraisal and coping for child caregivers. The study partially supported the effects of the problem-solving telephone counseling to improve the coping strategies, and these finding suggest the need for screening most vulnerable subgroups of caregivers first and then applying adequate and effective interventions for the subgroups.

국문 초록 (Abstract)

본 연구의 목적은 문제해결식 전화상담이 치매노인을 돌보는 가족간호제공자의 간호제공상황평가와 대처기전에 미치는 효과를 파악하기 위함이다. 2002년 1월부터 10월까지 P시에 거주하며 치매노인을 돌보고 있는 가족간호제공자를 무작위 추출하여(실험군 21명, 대조군 32명), 12주간의 문제해결식 전화상담을 실시한 후 집단간의 간호제공상황평가와 대처기전의 변화를 t-test로 분석하였다. 연구결과는 다음과 같다. 1)문제해결식 전화상담 후 실험군과 대조군의 간호제공상황평가와 대처기전에 대한 점수변화에서 두 집단간의 유의한 차이는 없었다. 2)문제해결식 전화 상담은 자녀간호제공자의 간호제공 상황평가와 대처기전에 영향을 미치지 않았으나 배우자간호제공자의 경우 실험군이 대조군보다 문제중심적 대처기전을 전화상담중재 후 유의하게 높게 활용하는 것으로 나타났다. 가족간호제공자를 위한 문제해결식 전화상담은 활용자원이 제한되어있고 취약한 특성을 가진 가족간호제공자에게 더 효과적일 수 있으며 간호중재의 모색에서 그 적용대상자의 특성이 우선적으로 고려될 필요가 있다.
