고령사회를 대비한 노인요양시설의 건강증진적 실내환경특성 연구

국문 초록 (Abstract)

본 연구는 신체적, 심리적, 사회적으로 열등하기 쉬운 노인들의 욕구를 충족시켜주며 건강을 증진시켜줄 수 있는 제반 환경특성을 살펴봄으로써 우리나라 노년층의 안녕과 복지를 도모하는데 궁극적인 목적을 갖고 있다. 본 연구의 사례대상은 노령인구비율이 높고 오랫동안 노령화 현상을 겪어 오면서 많은 지식기반 및 사례들을 축적해 온 스웨덴의 노인보호주택으로 하였다. 문헌연구 과정에서는 건강의 개념, Salutogenesis 개념과 특성, 건강증진적 환경과 관련된 연구결과들을 중심으로 살펴보았다. 연구목적에 적합한 구체적 사례조사대상을 선정하기 위해 스웨덴 노인보호주택 사례를 소개한 책자와 Research Center Design & Health 에서 2004년 실시한 노인보호주택 연구를 근거로 하였다. 연구조사한 노인보호주택은 ‘Slottsovalen’, ‘Säbyhemmet’과 Ystad에 소재한 ‘Vigs Ängar’이며 본 조사는 2005.9.-10.에 이루어졌다. 조사내용은 1)노인보호주택의 위치 및 지역사회와의 연계성 2) 공동공간의 종류, 갯수 3) 개인공간의 종류, 갯수 4)옥외정원 및 자연과의 접근성 5)거주자의 일반사항 6) 거주자의 건강상태 7) 거주자의 건강에 영향을 미치는 요인 등이였으며 사진촬영, 설문조사, 담당자와의 인터뷰, 평면도확보 등을 통해 이루어졌다. 본조사를 통해 얻은 노인보호주택의 특성들을 살펴보면 다음과 같다.
첫째, 노인보호주택 Slottsovalen의 특성은 크게 1) 주변의 좋은 자연환경시설과 함께 건물 중정에는 치료적인 요소들을 고려한 옥외정원이 있다. 2) 가족지향적인 생활을 기본 철학으로 하고 있다. 3) 건물 전체의 분위기를 밝게 하기 위해 명도가 높은 색채와 밝은 조명을 계획하였다. 둘째, 노인보호주택 Säbyhemmet 은 크게 4개의 유닛으로 구성되어 있으며 각 유닛당 8-10명의 노인들이 거주할 수 있도록 되어 있으며 침상수는 총 36개로 모두 일인용 침실로 구성되어 있다. 그 특징은 1) Säbyhemmet 은 모든 편익시설, 금융시설이 있는 City Center가 가까이에 위치해 있어서 이를 잘 활용하고 있다. 2) 기존 지역사회와의 긴밀한 연대감을 갖고 있어서 노인들의 심리적, 사회적 건강에 긍정적 영향력을 미치고 있다 3) 어린이 보육센터와의 적극적인 연계프로그램을 통해 노인들은 긍정적인 자극을 받을 수 있으며 정신적, 심리적 건강에 좋은 영향을 주고있다. 4) Säbyhemmet 직원과 노인과는 1:1 관계시스템을 갖고 있다. 셋째, 노인보호주택 Vigs Ängar 는 ’Antroposophic philosopy’ 에 영향을 받아 계획되어 건물, 노인, 직원을 자연의 부분으로 간주하였으며 전체 평면을 생태학적 사이클 모형을 본따서 계획하였다. 특징으로는1) 단층건물로 된 Vigs Ängar 의 전체 평면은 생태학적 사이클이 연상되는 고리형태로 되어 있다. 2) 다양성이라는 요소로 건물의 단조로움에 변화를 주려고 하였다. 3) 건축적 암시로 각 공간의 성격을 추측할 수 있도록 하였다. 4) Vigs Ängar 는 노인들의 잔존능력을 지속적으로 사용하도록 도와주고 있다.
이러한 연구결과를 근거로 하였을 때 건강증진적인 노인보호주택 환경이 되기 위해서는 ‘통합’ (Intergration), ‘집과 같은 환경’ (Homelike Environment), ‘가족지향적인 생활추구’ (Family-oriented Life), ‘자연과의 접근성’ (Accessibility to Nature), '하드웨어와 소프트웨어의 적절한 역할수행' 등으로 결론을 내릴 수 있다.

다국어 초록 (Multilingual Abstract)

This research is to identify the health-improving environmental characteristics for the elderly through field study. Three elderly care homes in Sweden visited were; Slottsovalen in Gustavsberg, Säbyhemmet in Rönninge, and Vigs Ängar in Köpinebro. Field studies were started from 14th September to 12th October 2005.

The results of this research are as follows; the main fators to affect positively elderly health in Slottsovalen are i) accessibility to nature and therapeutic yard, ii) a family-oriented life iii) a bright color and bright light. In Säbyhemmet case, there are i) lively interaction with current community ii) connected programmes or events with family and relatives or children at daycare center iii) 1:1 contact personal system between staff and elderly. In Vigs Ängar case, there are i) Antroposophic philosopy as a basic concept ii) variety among simple physical environment iii) architectual hint to indicate a function of room iv) encouragement to use an existing ability of eldelry.

According to this research, the essential factors to improve the health and well-being for the elderly are as follows;
1) Intergration: Be intergtated into current community and elderly care home, 2) Homelike environment: Homelike environment works as a therapeutic instrument to strengthen a cognitive ability and provide a safety feeling. Good relationship between eldelry and staff like as a family, avoiding long corridors, open connection between kitchen and dining area, resident’s own furniture and personal belongings are homelike items, 3) Accessibility to garden and nature: Garden offers a number of stimuli and opportunities for distraction from the wandering drive 4) Family-oriented life makes the eldelry the sense of safety, comfortability and well-being. 5) the role of hardware and software.
