흉부 물리치료는 와상 환자, 특히 폐렴 및 무기폐 위험이 높은 65세 이상 고령 환자의 호흡 재활에 중요한 역할을 한다. 그러나 기존의 수동 흉부 타진은 노동 집약적이고 표준화가 부족하여 치료 결과의 일관성이 떨어진다. 본 연구는 와상 환자를 위한 표준화된 흉부 물리치료를 제공하는 자동화 흉부 진동 로봇 시스템을 설계하고 검증하여, 일관된 치료 품질을 보장하는 동시에 의료진의 신체적 부담을 줄이는 것을 목표로 한다. 2자유도 리니어 갠트리와 6자유도 매니퓰레이터(MyCobot 320)로 구성된 8자유도 하이브리드 로봇 시스템을 개발하였다. 이 시스템은 MediaPipe Pose 추정을 사용한 환자 신체 인식 및 위치 지정을 위한 RGB-D 카메라(Astra Mini S), 실시간 힘 피드백 제어를 위한 FSR(Force Sensing Resistor) 센서, 그리고 10-15Hz 주파수의 흉부 진동을 전달하는 진동 모듈을 통합하였다. 목표 힘(5N)을 안전 허용 범위(±1N) 내에서 유지하기 위해 지수 이동 평균(EMA) 필터링을 적용한 폐쇄 루프 임피던스 제어 알고리즘을 구현하였다. 시스템은 6개 흉부 위치에서 3명의 피험자를 대상으로 카메라 기반 위치 추정 정확도 테스트와 힘 피드백 실험을 통해 검증되었다. 카메라 기반 위치 추적은 보정 후 모든 축에서 평균 오차 0.01m 미만의 높은 정확도를 달성하였다. 로봇 시스템은 목표 지점의 83.3%에서 ±5mm 허용 오차 내에 위치를 유지하였다. 힘 피드백 제어는 피험자 전체에서 평균 적용 힘이 3.64N에서 4.55N 범위로 일관된 성능을 보였으며, 가장 우수한 피험자는 측정값의 61.4%가 목표 범위인 4-6N 내에 있었다. 개발된 자동화 흉부 진동 로봇 시스템은 표준화되고 안전하며 일관된 흉부 물리치료를 제공할 수 있는 가능성을 성공적으로 입증하였다. 비전 기반 환자 위치 지정, 실시간 힘 피드백 제어 및 안전 메커니즘의 통합은 환자 안전을 유지하면서 정밀한 치료 전달을 가능하게 하며, 임상 호흡 재활 환경에서 치료 품질 향상에 대한 가능성을 보여준다.

Chest physiotherapy plays a crucial role in respiratory rehabilitation for bedridden patients, particularly elderly patients aged 65 and above who are at high risk of pneumonia and atelectasis. However, conventional manual chest percussion is labor-intensive and lacks standardization, resulting in inconsistent treatment outcomes. This study aims to design and validate an automated chest vibration robotic system that provides standardized chest physiotherapy for bedridden patients, ensuring consistent treatment quality while reducing the physical burden on healthcare workers. An 8-degree-of-freedom (DOF) hybrid robotic system was developed, comprising a 2-DOF linear gantry and a 6-DOF manipulator (MyCobot 320). The system integrates an RGB-D camera (Astra Mini S) for patient body recognition and positioning using MediaPipe Pose estimation, Force Sensing Resistor (FSR) sensors for real-time force feedback control, and a vibration module delivering chest vibrations at 10-15 Hz frequency. A closed-loop impedance control algorithm with Exponential Moving Average (EMA) filtering was implemented to maintain the target force (5N) within a safe tolerance range (±1N). The system was validated through camera-based position estimation accuracy tests and force feedback experiments with three subjects at six chest positions. Camera-based position tracking achieved high accuracy with mean errors below 0.01 m across all axes after calibration. The robotic system maintained positioning within ±5 mm tolerance at 83.3% of target points. Force feedback control demonstrated consistent performance with mean applied forces ranging from 3.64N to 4.55N across subjects, with the best-performing subject achieving 61.4% of measurements within the target range of 4-6N. The developed automated chest vibration robotic system successfully demonstrates the feasibility of providing standardized, safe, and consistent chest physiotherapy. The integration of vision-based patient positioning, real-time force feedback control, and safety mechanisms enables precise treatment delivery while maintaining patient safety, showing promise for improving treatment quality in clinical respiratory rehabilitation settings.
