가족치료 사례에서 나타난 생애주기별 신경성 폭식증의 진행단계와 가족요인에 관한 질적 연구 = Life-Cycle Progressive Stages and Familial Factors of Bulimia Nervosa Observed in a Family Therapy Case

국문 초록 (Abstract)

본 연구의 목적은 신경성 폭식증을 가진 내담자에 대한 가족치료 사례를 중심으로 생애주기별 신경성 폭식증의 진행단계와 신경성 폭식증에 영향을 미친 가족요인을 살펴보는데 있다. 본 연구는 총 11회기 동안 내담자와 가족을 대상으로 진행된 상담축어록을 중심으로 질적 분석방법을 활용하여 주요개념을 도출하였다. 본 연구결과 생애주기별 신경성 폭식증의 진행단계는 영유아기 애착불안 형성단계, 아동기 자아의 억압에 따른 심리적 허기단계, 청소년기 스트레스와 심리적 갈등의 증폭단계, 그리고 성인초기 부정적 감정해소를 위한 보상적 행위단계로 분석되었다. 또한 신경성 폭식증에 영향을 미친 가족요인의 특성으로는 만성적 부부갈등과 불안한 분위기, 정서적 미분화와 희생양 역할, 정서적 불안과 불만을 야기하는 모의 독단적 의사소통방식으로 분석되었다. 이와 같은 연구결과는 신경성 폭식증이 생애주기에 따라 점진적인 진행단계를 거쳐 발생하므로 가족치료 과정에서 내담자가 생애주기 동안 경험한 유의미한 사건과 문제를 통합적으로 고려해야 할 필요성을 보여주었다. 또한 본 연구결과는 신경성 폭식증의 발생요인에 대한 개인적 접근뿐만 아니라 가족체계적 접근의 필요성과 신경성 폭식증으로 어려움을 겪는 내담자와 가족을 위한 임상적 개입방안을 제시하였다.

다국어 초록 (Multilingual Abstract)

The purpose of this study is to investigate the life cycle progressive stages and the familial factors influencing on the client’s bulimia nervosa, based on a family therapy case study. The main concepts were derived by using qualitative analysis method focusing on the client’s and family’s counseling verbatim for a total of 11 sessions. The bulimia nervosa was found to undergo a certain stage throughout the life cycle: anxiety attachment stage in infancy, psychological emptiness stage due to self-suppression in childhood, stress and psychological conflict amplification stage in adolescence, and compensational behavior stage for relieving negative emotions in adulthood. In addition, the client’s bulimia nervosa was influenced by several familial factors such as chronic marital conflicts, unstable domestic atmosphere, undifferentiated emotion, role as a scapegoat, and lastly, the mother’s dogmatic communication pattern which caused anxiety and grievance. The results indicate that it is necessary to take into account the meaningful events and problems experienced throughout the client’s life cycle and the family therapy process since bulimia nervosa gradually develops throughout each stages. The results of this study also suggest the need for family oriented approach as well as individual approach to bulimia nervosa and clinical interventions for clients and families suffering from these difficulties.
