Most cases of cytomegalovirus (CMV) colitis occur in adults with severe immuno- deficiency. There have been a few reports involving immunocompetent patients. CMV colitis may occur after colonic mucosal injury in immunocompetent patients. Henoch-Schönlein purpura (HSP) is a common systemic vasculitis in childhood. Diagnostic criteria include palpable purpura with at least one other manifestation; abdominal pain, IgA deposition, arthritis or arthralgia, or renal involvement. To best of our knowledge, we describe the first case of CMV colitis in an immunocompetent patient with preceding HSP. A 38-year-old man presented with a 1-day history of abdominal pain, diarrhea and vomiting. Two-years previously, he underwent a right hemicolectomy due to intussusception. Over a 1-month period, palpable purpura was evident on both arms and legs. Colonoscopy revealed multiple, linear geographic ulcerations at an anastomosis opening site with huge ulceration at the small bowel, which was covered with white exudates. Colonoscopic biopsy showed CMV inclusion bodies and skin biopsy revealed leukocytoclastic vasculitis. The patient was successfully treated with gancyclovir and prednisolone.

국문 초록 (Abstract)

거대 세포 바이러스 대장염은 주로 면역이 저하된 환자에서 나타나며 면역 기능이 정상인 환자에서는 대장 점막의 손상 이후에 발생한다는 보고가 있다. Henoch-Schönlein 자반증(HSP)은 IgA 면역 복합체와 관련되어 소혈관을 주로 침범하는 백혈구 파괴성 혈관염의 일종으로 촉진성 자반증, 관절염, 복통, 신장 침범 등이 특징인 질환이다. 현재까지 HSP와 함께 거대 세포 바이러스 감염이 동반된 보고는 없었다. 38세 남자가 내원 1일 전부터 시작된 복통과 설사, 구토를 주소로 내원하였다. 환자는 2년 전 지방종에 인한 장중첩증으로 우측 반 대장 절제술을 시행 받았으며, 한 달 전부터 상하지에 촉진성 자반이 발생하였다. 대장 내시경 검사에서 수술 문합부와 소장 연결 부위에 다수의 선상 및 지도상 궤양, 충혈, 백색 삼출물 소견이 관찰되었으며 소장 내부에도 다수의 궤양이 관찰되었다. 조직 검사에서 육아종을 동반한 궤양, 전형적인 거대 세포 바이러스 봉입체가 관찰되어 거대 세포 바이러스 장염으로 진단하였다. 피부에서 시행한 조직 검사에서 백혈구 파괴성 혈관염 소견이 보였고 복통과 혈뇨의 임상 양상을 종합하여 HSP로 진단하였다. 환자는 gancyclovir, prednisolone으로 치료하면서 증상이 호전되어 퇴원 후 외래에서 추적 관찰 중이다. 저자들은 정상 면역을 가진 환자에서 HSP와 동반된 거대 세포 바이러스 장염 1예를 경험하여 보고한다.
