척추 후방기기 고정후 전방유합술과 후측방유합술의 전향적 비교 = Prospective Comparison between the Anterior Interbody Fusion and the Posterolateral Fusion

국문 초록 (Abstract)

목적 : 후측방유합술과 추체간 전방유합술간의 임상적, 방사선학적 결과를 비교하고자 하였다.
대상 및 방법 : 불안정성을 동반한 추간판 탈출증, 척추 협착증, 척추 전방 전위증과 추간판 내장증으로 진단되어 척추 유합술이 요구되는 환자를 대상으로, 전향적 연구를 시행하기 위하여 제 1군은 후방기기술 및 추체간 전방유합술을 시행한 20명의 환자로, 제 2군은 후방기기술 및 후측방유합술을 시행한 20명의 환자로 분류하여 수술시간 및 수술전후의 혈액학적 변화와 기능회복 정도, 골유합률의 차이를 분석하였다.
결과 : 1군과 2군의 평균 수술시간은 각각 180분과 100분이었으며, 수혈량과 총 실혈량의 평균치는 1군이 926cc, 1225cc, 2군이 880cc, 1034cc 이었다. 임상 결과에 있어서 제 1군의 경우 우수 25%, 양호 65%, 저효 10% 였으며, 제 2군의 경우 우수 15%, 양호 50%, 저효 30%, 무효 5%를 보인 것으로 관찰되었다. 골유합률은 제 1군의 경우 전례에서, 제 2군의 경우 75%에서 골유합 소견이 관찰되었다.
결론 : 골유합률 및 임상적 만족도에서 후방기기술 및 추체간 전방유합술이 후측방유합술에 비하여 우수한 결과를 보였다.

다국어 초록 (Multilingual Abstract)

Objectives : To campare the differences of clinical and raiological results berween the posterolateral fusion and the anterior interbody fusion.
Materials and Methods : For prospective study, the anterior interbody fusion was performed in 20 patients(Group Ⅰ)and the posterolateral fusion was performed in 20 patients(Group Ⅱ). The operation time, pre-& post-operative hemodynamic changes, functional recovery and fusion rate were compared.
Results : The average operation time was 180 min in group Ⅰ and 100 min in group Ⅱ. The average estimated blood loss and transfusion amount were assessed 1225cc,926cc in group Ⅰ and 1034cc, 880cc in group Ⅱ. Clinical results weere as follows: excelient 25%, good 65%, fair 10% in group Ⅰ and excelent 15%, good 50%, fair 30%, poor 5% in group Ⅱ. The fusion rate was 100% i group Ⅰ and 75% in group Ⅱ.
Conclusion : The anterior nterbody fusion with posterior instrumentation is considered encouraging operative methods.
