노인의 의료서비스 이용에 대한 종단적 연구 = A Longitudinal Study on Older Adults’ Use of Medical Services

국문 초록 (Abstract)

본 연구에서는 뉴만(Newman)과 앤더슨(Andersen)의 행동주의모델을 이론적 분석틀로 노인의 의료서비스 이용에 영향을 미치는 요인을 분석함을 목적으로 한다. 분석에 사용된 자료는 「고령화와 한국노인의 삶의 질에 관한 연구」의 3개년도의 패널자료이며, 노인의 의료서비스 이용은 입원여부와 외래진료여부와 나누어서 살펴보았다. 자료분석은 일반화 추정 공식(Generalized Estimating Equations: GEE)을 이용하였다. 입원여부에 대한 분석결과, 기초선(baseline) 연령이 낮을수록, 주관적 건강상태가 나쁠수록, 만성질환의 수가 많을수록, 그리고 인지능력이 낮을수록 입원할 경향이 증가하였다. 또한 외래진료여부에 대한 분석결과, 시간이 증가함에 따라, 기초선 연령이 낮을수록, 의료급여대상자일수록, 도시지역에 거주할수록, 주관적 건강상태가 나쁠수록, 만성질환의 수가 많을수록, 인지능력이 높을수록 외래진료를 이용할 경향이 증가하였다. 결론에서는 노인집단내 의료서비스 이용의 불공평한(inequitable) 접근을 줄이기 위한 정책적 시사점에 대하여 논하였다.
