노인 암 환자의 건강보험이용과 영향요인 = The Factors Influencing and Characteristics of National Health Insurance Utilizations among the Elderly Cancer Patients.

다국어 초록 (Multilingual Abstract)

The purposes of this paper were to examine the characteristics of national health insurance(NHI) utilizations among the elderly cancer patients, and to identify factors influencing NHI utilizations of serious disease cancer patients. First, the NHI utilizations of the elderly cancer patients were few than the young. It was different with the utilizations of total diseases. The elderly cancer patients who economic power are few, were passive the tendency of utilizations on expensive medical services like cancer. Second, the elderly cancer patients who ages are low, sex is man and health care system is medical aid program, have few NHI utilizations. Third, the elderly cancer patients who are using big medical institution in special city, have many NHI utilizations. Fourth NHI utilizations of the elderly cancer patients were influenced meaningly by the characteristics of medical institutions and patients. Hereafter, we must make the elderly cancer patients to used adequate health services through the reduction of co-payment and improvement of accessibility for medical institutions.

국문 초록 (Abstract)

본 연구는 중증질환인 노인 암환자의 건강보험이용 특성을 분석하고, 영향요인을 분석하는데 목적이 있다. 주요 분석결과는 다음과 같다. 첫째, 노인 암환자의 건강보험이용은 일반적인 의료이용과는 달리 젊은 층에 비해서 적은 것으로 나타났다. 경제력이 낮은 노인들이 비용이 많이 드는 암 등 중증질환의 경우에는 치료에 소극적인 것이다. 둘째, 저연령환자, 남성환자, 저소득층인 의료급여환자, 중증도가 높은 노인 암환자들의 건강보험이용량이 많았다. 셋째, 특별시에 개설된 의료기관과 대형의료기관을 이용하는 노인 암환자들의 건강보험이용량이 많았다. 넷째, 환자특성과 의료자원특성이 노인 암환자의 건강보험이용에 유의미한 영향을 미치는 것으로 나타났다. 향후 노인 암환자의 본인부담을 인하하고, 의료기관에 대한 접근성 향상으로 적절한 의료서비스를 이용할 수 있도록 해야 한다.
