정신병원, 종합병원, 대학병원에서 정신분열병 환자의 입원치료에 대한 만족도 비교 = Comparison of Satisfaction with Inpatient Psychiatric Treatment between Psychiatric Hospital, General Hospital, and University Hospital in Patients with Schizophrenia

국문 초록 (Abstract)

목 적
정신병원, 종합병원 정신과, 대학병원 정신과의 입원병동에서 퇴원하는 정신분열병 환자들을 대상으로 입원치료에 대한 만족 도를 조사함으로써, 정신의료기관의 유형에 따른 차이와 입원치료 만족도에 영향을 미치는 유의한 변수를 알아보기 위함이다.
방 법
정신병원, 종합병원, 대학병원 정신과 병동에서 증상이 호전되어 퇴원하는 정신분열병 환자 323명을 대상으로 신뢰도와 타당 도가 입증된 입원서비스 만족척도를 이용하여 입원치료에 대한 만족도를 조사하였다.
결 과
입원서비스 만족척도의 전체점수에서 대학병원 환자군, 종합병원 환자군, 정신병원 환자군 순으로 각각 유의하게 높았다. 입원서비스 만족척도의 5개 하위척도 중에서 치료진 태도, 치료의 질, 병동환경, 병동규칙에서는 전체점수와 마찬가지로 대학병원 환자군, 종합병원 환자군, 정신병원 환자군 순으로 만족점수가 유의하게 높았지만, 접근성/비용 하위척도에서는 세 군간에 유의한 차이가 없었다.
성별, 나이, 학력, 직업, 월수입, 종교, 결혼상태, 입원유형, 유병기간, 입원횟수, 입원일수가 입원서비스 만족척도의 전체점수에 미치는 영향을 알아보기 위하여 11개 변수를 입원서비스 만족척도의 전체점수에 대한 설명변수로 고려하여 분산분석을 실시한 결과 p<.001 이하로 유의하였고(F 9,313 =14.02) 결정계수(R 2 )는 0.29였다. 그리고 p<.05에서 유의한 설명변수들은 정신의료기관 유형, 입원유형, 월수입이었다.
정신병원, 종합병원, 대학병원에서 만족도 율이 높은 문항들은 공통적으로 담당의사와 간호사의 태도와 능력에 관한 것이었지 만, 불만족 율이 높은 문항들에서는 차이를 보였다. 정신병원과 종합병원에서는 병동규칙이나 환경에 대한 불만족 율이 높은 반면 대학병원에서는 치료비용과 입퇴원에 대한 치료진의 설명에 대해 불만족 율이 높았다.
결 론
입원서비스에 대한 만족도에서는 대학병원, 종합병원, 정신병원 순이었고, 만족도에 유의한 영향을 미치는 변수로는 정신의료 기관 유형, 입원유형, 월수입이었다. 따라서 정신분열병 환자에서 입원치료에 대한 만족도를 높이기 위해서는 강제입원보다는 자발적 입원을 유도해야 할 필요성이 있다.

다국어 초록 (Multilingual Abstract)

Objective：The purpose of this study is to compare the schizophrenic inpatient satisfaction with hospital treatment between the psychiatric hospital, general hospital, and university hospital and to investigate the variables that influence the inpatient satisfaction with hospital treatment.
Methods：A 30-item multidimensional questionnaire tested for good validity and reliability was administered to inpatients with schizophrenia(n=323) discharged from psychiatric hospitals, general hospitals, and university hospitals.
Results：Patients discharged from university hospital were significantly satisfied than those of general and psychiatric hospital in the total scores of service satisfaction scale. Patients with university hospital were significantly satisfied than those of general and psychiatric hospital in the subscales of staff attitude, treatment quality, ward environment, ward policy, and there were no differences in the subscale of access/cost between psychiatric hospital, general hospital, and university hospital.
Three variables such as type of hospital, type of admission, monthly income were significant explanation on the satisfaction with inpatient psychiatric services.
The common items with high percentage of satisfaction were included the attitude and ability of doctor or nurse in all three type of hospitals. But high percentage dissatisfied for the items were different between psychiatric facilities.
Patients discharged from general and psychiatric hospital expressed high dissatisfaction with the ward rule or environment, but those discharged from university hospital expressed high dissatisfaction with treatment cost and the staff's explanation for hospitalization. Staff attitude is the most important determinant in the satisfaction of psychiatric inpatients.
Conclusion：Patients discharged from university hospital were significantly satisfied than those of general and psychiatric hospital in the total scores of service satisfaction scale. Type of hospital, type of admission, monthly income were significant explanation variables on the satisfaction with inpatient psychiatric services. For increasing the satisfaction level of psychiatric inpatient in Korea, it is necessary to induce patient to hospitalize voluntarily.
