    With changes in demographic structures, the number of single-person households is rapidly increasing worldwide. The proportion of single-person households within the disabled population is also on the rise. Individuals with disabilities living alone are situated at the intersection of the vulnerabilities inherent in single-person households and the limitations imposed by disability. As such, they are likely to be at a disadvantage, particularly in terms of healthcare access. “Unmet healthcare needs” serve as a representative indicator of healthcare access, and individuals with disabilities tend to report higher rates of unmet healthcare needs compared to those without disabilities. Therefore, this study aims to identify the factors associated with unmet healthcare needs among individuals with disabilities living alone, with the ultimate goal of contributing to the provision of foundational data for improving their access to healthcare services.

    This study utilized data from the 2020 and 2023 National Survey of Disabled Persons, focusing on single-person households with disabilities under the age of 85. In order to identify the factors associated with their experiences and the causes of unmet medical needs, binary logistic regression and multinomial logistic regression analyses were conducted. Furthermore, by analyzing the 2020 data (during the COVID-19 outbreak period) and the 2023 data separately, the study examined changes in the experience of unmet medical needs among individuals with disabilities living alone over time.

    During the COVID-19 outbreak period, individuals residing in non-metropolitan areas, those with lower income levels, those with chronic conditions, those with limited access to health information, those who experienced discrimination when using medical institutions, and those not receiving personal assistance services were more likely to report unmet healthcare needs. In the 2023 data, unmet healthcare needs were more likely among individuals with lower income levels, physical disabilities affecting mobility, longer durations of disability, higher IADL(Instrumental Activities of Daily Living) scores, limited access to health information, and those not using personal assistance services.

    The most notable temporal differences were pronounced in the variables of “residential area,” “presence of chronic disease,” and “experience of discrimination when using medical institutions.” These variables were statistically significant only during the COVID-19 outbreak period, indicating that the access to healthcare for individuals with disabilities living alone declined in a manner distinct from normal circumstances during a public health crisis. Some medical institutions prioritized infectious disease control by converting themselves into COVID-19 dedicated hospitals or shutting down entirely. At the same time, disparities in healthcare infrastructure between metropolitan and non-metropolitan areas became evident, leading to a higher likelihood of unmet healthcare needs among residents in non-metropolitan regions. Additionally, individuals with chronic conditions who typically have a higher and more consistent need for medical services were more likely to experience unmet healthcare needs. The experience of discrimination in medical institutions indicates a lack of adequate social consideration for vulnerable populations during the public health crisis.

    This study is significant in that it is the first study to examine the current state of healthcare access among individuals with disabilities living alone—an issue that has not been sufficiently highlighted in social and policy discussions. In particular, by conducting a temporal analysis, the study identifies the factors that hinder healthcare access for individuals with disabilities living alone in the context of a public health crisis. The findings confirm that multidimensional factors influence the occurrence of unmet medical needs among this population. Moreover, the impact of each factor varies depending on the specific causes of unmet healthcare needs.

    Given the expansion of community-based independent living support and de-institutionalization policies, the number of individuals with disabilities living alone is anticipated to steadily increase. Accordingly, it is essential that the future health policies take into full account both the unique characteristics of this population and the specific factors that contribute to barriers in healthcare access. Based on this understanding, it is hoped that a social environment can be fostered in which the health rights of individuals with disabilities living alone are sufficiently protected.

    인구구조의 변화와 함께 1인 가구는 전 세계적으로 빠른 속도로 증 가하고 있다. 장애인구 집단 내 1인 가구 비중 또한 증가 추세에 있다. 장애인 1인 가구는 장애로 인한 제약과 1인 가구가 내포하고 있는 취약성이 교차하는 지점에 놓인 집단으로, 특히 의료접근성 측면에서 상대적으로 불리한 위치에 자리할 가능성이 높다. ‘미충족의료’는 의료접근성을 대표하는 지표로, 장애인은 비장애인에 비해 높은 미충족의료 경험률을 보인다. 이에 본 연구는 ‘장애인 1인 가구’의 미충족의료 경험에 영향을 미치는 요인을 파악함으로써, 이들의 의료접근성 향상을 위한 기초자료를 제공하는 것을 목적으로 한다.

    본 연구에서는 장애인실태조사 2020년, 2023년 자료를 활용하여 만 85세 미만 장애인 1인 가구를 대상으로 이들의 미충족의료 현황을 파악하였다. 이들의 미충족의료 경험 여부와 원인에 영향을 미치는 요인을 확인하고자 이항 로지스틱 회귀분석과 다항 로지스틱 회귀분석을 수행하였다. 아울러 장애인실태조사 2020년(코로나19 확산 시기), 2023년 자료 를 구분하여 분석함으로써, 시기별 장애인 1인 가구의 미충족의료 경험의 변화 양상을 살펴보았다.

    먼저 코로나19 확산 시기에는 비수도권 거주자일 경우, 가구 소득분위가 낮은 경우, 만성질환 보유자일 경우, 건강정보에 접근이 불가능할 경우, 의료기관 이용 시 차별경험이 있을 경우, 활동지원 서비스를 이용하지 않을 경우 미충족의료를 경험할 가능성이 높았다. 이후, 2023년 자료를 통한 분석에서는 가구 소득분위가 낮은 경우, 외부 신체기능 장애인일 경우, 장애 보유기간이 길수록, IADL 점수가 높을수록, 건강정보에 접근이 불가능할 경우, 활동지원 서비스를 이용하지 않을 경우 미충족의료를 경험할 가능성이 높았다.

    시기별 차이는 ‘거주지역’, ‘만성질환 보유 여부’, ‘의료기관 이용 시 차별 경험’에서 극명하게 드러났다. 이는 코로나19 확산 시기에서만 통계적으로 유의하게 나타난 변수로, 공중보건 위기 상황에서 장애인 1인 가구의 의료접근성은 일상과는 다소 다른 양상으로 저하되었음을 보여준다. 해당 시기에 일부 의료기관은 감염병 대응을 우선하며 코로나19 전담 병원으로 전환하거나 병원을 폐쇄하기도 하였으며, 무엇보다 수도권과 비수도권 사이 의료인프라 격차가 드러나면서 비수도권에 거주할 경우 미충족의료를 경험할 가능성이 높게 나타났다. 더불어 의료에 대한 지속적인 요구가 높은 만성질환 보유자일 경우 미충족의료를 경험할 가능성 이 높았다. 나아가 의료기관 이용 과정에서 장애인 1인 가구가 경험한 차별은 공중보건 위기 상황에서 취약계층의 사회적 고려가 부족했음을 시사한다.
