항암화학요법을 받는 암환자의 통증, 우울, 암 재발 두려움이 수면의 질에 미치는 영향 = Impact of Pain, Depression, Fear of Cancer Recurrence on Sleep Quality in Cancer Patients with Chemotherapy

The purpose of this study was to identify pain, depression, fear of cancer recurrence might influence the sleep quality in cancer patients receiving chemotherapy. Subjects were 129 cancer patients who received chemotherapy and visited the outpatient clinic 1 week after discharge at a University Hospital located in C city. Data were collected from March to August 2021, using Numerical Pain Rating Scale, Zung Self-rating Depression Scale, the Fear of Cancer Recurrence Inventory, and Pittsburgh Sleep Quality Index, The data were analyzed by SPSS WIN 24.0. The main results are as follows. There were significant differences in the sleep quality according to gender, cancer recurrence, diagnosis, and duration of cancer. There were significant correlations between pain, depression, fear of cancer recurrence, and sleep quality. As a result of hierarchical multiple regression, fear of cancer recurrence, gender, recurrence, and duration of cancer were found to be significant influencing factors. Therefore, psychosocial intervention such as cognitive behavioral therapy and mindfulness should be provided. In addition, there is a need for a policy to expand manpower to provide psychosocial intervention and to establish a psycho-oncology department in a cancer center.

국문 초록 (Abstract)

본 연구의 목적은 항암화학요법을 받는 암환자의 통증, 우울, 암 재발 두려움이 수면의 질에 미치는 영향을 파악하는 것이다. 자료수집은 2021년 3월 1일부터 8월 31일까지 C시에 소재한 일개 대학병원에서 항암화학요법을 받고 퇴원한 지 일주일 후에 외래에 방문한 환자 129명을 대상으로 하였다. 연구도구는 통증, 우울, 암재발 두려움, 수면의 질을 사용하였고, SPSS WIN 24.0을 이용하여 자료를 분석하였다. 연구결과, 성별, 암 재발 여부, 진단명, 암 유병기간에 따라 수면의 질에 유의한 차이가 있었고, 통증, 우울, 암 재발 두려움도 수면의 질과 유의한 상관관계가 있는 것으로 나타났다. 위계적 회귀분석 결과, 암 재발 두려움과 함께 성별, 재발 여부, 암 유병기간이 수면의 질의 유의한 영향요인으로 나타났다. 따라서 항암화학요법을 받는 암 환자들의 수면의 질을 향상시키기 위해서는 인지행동치료와 마음챙김 등의 심리적 중재와 함께, 통증과 우울, 암 재발 두려움을 감소시키는 중재도 제공되어야 하며, 이를 위한 인력을 확중하고 암센터 내 심리치료 부서를 필수적으로 설치하도록 하는 정책이 필요하다.
