    한국은 2025년 초고령사회에 진입하였으며, 이에 노년층의 건강증진행위는 건강수명 연장과 만성질환 관리 측면에서 더욱 중요한 의미를 가지게 되었다. 노인의 건강증진행위에 영향을 미치는 여러 요인 중, COVID-19 이후 디지털 기반 건강정보 활용이 증가함에 따라 노인의 디지털 헬스 리터러시의 중요성이 강조되었으나 디지털 기기 사용과 온라인 정보의 선별·평가 및 실제 건강행동 적용 과정에서 여러 어려움을 경험하고 있다. 또한 노인의 건강증진행위는 단순한 정보 접근 능력만으로 설명되지 않으며, 실제로 정보를 탐색하고 활용하는 건강정보 추구행동이 핵심적 매개 역할을 할 수 있다. 또한 부부는 생활환경과 의사결정 과정을 공유하기 때문에 건강정보 탐색과 행동이 상호 영향을 주는 관계적 특성이 존재한다. 이에 본 연구는 노인 부부의 디지털 헬스 리터러시가 자신과 배우자의 건강증진행위에 건강정보 추구행동을 매개로 하여 영향을 주는지 쌍자료(dyadic data)를 이용하여 자기-상대방 상호의존 매개모형(Actor–Partner Interdependence Mediation Model, APIMeM)방법을 통해 분석하였다.
    본 연구는 지역사회 거주 60세 이상 노인 부부 204쌍(408명)을 대상으로 한 서술적 인과관계 연구이다. 주요 변수는 디지털 헬스 리터러시, 건강정보 추구행동, 건강증진행위였으며, 공변량으로 일반적 특성과 부부 의사소통 수준을 통제하였다. 자료는 SPSS/WIN 29.0과 R 4.5.1로 분석하였고, 매개효과의 유의성은 5,000회 부트스트래핑(bootstrapping)으로 검증하였다. 연구대상자의 디지털 헬스 리터러시 평균은 남편 28.34점, 아내 29.87점이었으며, 건강정보 추구행동은 남편 3.33점, 아내 3.36점이었다. 건강증진행위는 남편이 평균 12.74점, 아내가 13.97점으로 아내가 유의하게 높은 수준을 보였다. 또한 모형 검증에서 디지털 헬스 리터러시는 남편과 아내 모두에서 건강증진행위에 대한 직접효과는 유의하지 않았으나, 건강정보 추구행동을 매개로 한 간접효과는 유의하였다(남편 β=.19, p<.001; 아내 β=.08, p=.019). 건강정보 추구행동은 두 집단 모두에서 유의한 자기 효과를 보였다(남편 β=.38, p<.001; 아내 β=.21, p=.003). 상대방 효과는 직접경로에서는 유의하지 않았으나, 남편의 디지털 헬스 리터러시는 아내의 건강정보 추구행동을 매개로 아내의 건강증진행위에 유의한 간접효과를 보였다(β=.04, p=.041). 남편과 아내 모두 자신에 대한 유의한 총효과가 확인되었으며, 아내의 디지털 헬스 리터러시는 남편의 건강증진행위에 대해서도 유의한 총효과를 나타냈다. 전체 모형 적합도는 CFI=.917, TLI=.848, RMSEA=.073, SRMR=.061로 수용 가능한 수준이었다.
    본 연구는 디지털 헬스 리터러시가 건강정보 추구행동을 매개로 하여 건강증진행위에 영향을 주며, 특히 상대방의 디지털 헬스 리터러시가 본인의 건강정보 추구행동을 매개로 하여 본인의 건강증진행위에 영향을 주는 상대방 효과(Partner effect)를 확인하였다. 이는 부부 단위의 상호작용 분석을 통해 노년기 건강행동이 개인 요인뿐 아니라 관계적 요인이 영향을 미친다는 점을 의미한다. 본 연구는 초고령사회에서 디지털 기반 건강관리의 실천적 전략을 마련하는 데 기초자료로 활용될 수 있으며, 향후 부부 기반 건강증진 프로그램, 성별 특화 디지털 헬스 교육, 맞춤형 건강정보 제공 시스템 개발에 중요한 근거를 제공할 수 있다.

    As Korea moves into a super-aged society, understanding factors that shape health-promoting behaviors among older adults is increasingly important. The rapid growth of digital health information after COVID-19 has highlighted digital health literacy as a key determinant of how older adults search for and use health information. However, most prior studies have focused on individuals, overlooking the interdependence between spouses. Since older couples share daily environments and health-related decision-making, their information-seeking patterns and health behaviors may affect one another. This study examined actor and partner effects of digital health literacy on health-promoting behaviors among elderly couples and tested the mediating role of health information-seeking behavior using the Actor–Partner Interdependence Mediation Model.
    Data were collected from 204 community-dwelling couples aged 60 or older. Measures included digital health literacy, health information-seeking behavior, and health-promoting behavior, with general characteristics and marital communication controlled. Analyses were conducted using SPSS 29.0 and R 4.5.1 with 5,000-sample bootstrapping. digital health literacy scores averaged 28.34 for husbands and 29.87 for wives. Health information-seeking behavior averaged 3.33 for husbands and 3.36 for wives. Mean health-promoting behavior scores were 12.74 for husbands and 13.97 for wives, indicating higher engagement among wives. Digital health literacy was not directly associated with health-promoting behaviors for husbands or wives, but indirect effects through information seeking were significant for both. Information seeking showed strong actor effects, whereas partner effects were limited. A small but significant indirect husband-to-wife path was observed, while the wife-to-husband indirect path was marginal. Model fit indices (CFI = .917, TLI = .848, RMSEA = .073, SRMR = .061) indicated acceptable fit.
    These findings suggest that digital health literacy influences health behaviors primarily by promoting active information seeking. The partial cross-spousal pathways support the need for couple-based digital health education and interventions to improve healthy aging in super-aged societies.
